【摘要】：目的:研究維吾爾族孕婦亞臨床甲狀腺功能減退癥(SCH)的影響因素,提出防治對策。方法:選取2015年1月至2015年12月就診于烏魯木齊市婦幼保健院的且符合納入及排除標準的維吾爾族孕婦,采用自行設計的調查問卷對選取的產婦進行面對面調查,收集并分析維吾爾族SCH患者的相關指標,將所有資料建立數(shù)據庫,采用逐步向前法擬合多因素logistic回歸模型探索SCH相關因素。結果:3428例維吾爾族中,妊娠期SCH患者490例,檢出率為14.29%。通過逐步回歸篩選表明,維吾爾族孕婦年齡、體質指數(shù)(BMI)、血紅蛋白濃度、既往糖尿病病史對SCH的發(fā)生有影響(P0.05)。年齡大于30歲、BMI大于24kg/m~2、貧血及既往合并糖尿病病史的維吾爾族孕婦發(fā)生SCH的風險較高。結論:高齡、肥胖、貧血、糖尿病是影響維吾爾族孕婦發(fā)生妊娠期SCH的主要因素。應加強孕前、孕期的健康宣教,避免高齡懷孕,控制孕期體質量,積極開展常見疾病監(jiān)測及診治,降低妊娠期SCH發(fā)生率。
[Abstract]:Objective: to study the influencing factors of subclinical hypothyroidism (Sch) in Uygur pregnant women and put forward the preventive and therapeutic measures. Methods: the pregnant women of Uygur nationality who met the criteria of inclusion and exclusion were selected from January 2015 to December 2015 in Urumqi City Maternal and Child Health Hospital. The self-designed questionnaire was used to conduct a face-to-face survey on the selected women. The related indexes of Uygur patients with sch were collected and analyzed, and all the data were established. The stepwise forward fitting multi-factor logistic regression model was used to explore the related factors of sch. Results of 3428 Uygur women, 490 cases had SCH during pregnancy, and the detection rate was 14.29%. Stepwise regression screening showed that age, body mass index (BMI), hemoglobin concentration and history of diabetes had influence on the occurrence of Sch in Uygur pregnant women (P0.05). The risk of SCH in Uygur women over 30 years old was higher than that in Uygur women with anemia and previous history of diabetes. Conclusion: old age, obesity, anemia and diabetes are the main factors that affect the gestational SCH in Uygur pregnant women. It is necessary to strengthen health education before pregnancy, avoid pregnancy at old age, control body mass during pregnancy, carry out surveillance and diagnosis of common diseases, and reduce the incidence of SCH during pregnancy.
