不同分娩方式與產(chǎn)后早期盆底肌電值、肌力變化關(guān)系研究

【摘要】:目的探討不同分娩方式與產(chǎn)后早期盆底肌肉肌電值、肌力變化的關(guān)系。方法選擇2014年1月至2015年1月妊娠36~38周于中國(guó)醫(yī)科大學(xué)附屬盛京醫(yī)院門診產(chǎn)檢、于盆底治療中心行產(chǎn)前產(chǎn)后盆底功能檢測(cè)、并符合納入標(biāo)準(zhǔn)與排除標(biāo)準(zhǔn)的329例產(chǎn)婦為研究對(duì)象,按照分娩方式不同將所有產(chǎn)婦分為選擇性剖宮產(chǎn)組162例與陰道分娩組167例,其中陰道分娩組分為會(huì)陰側(cè)切組84例與非側(cè)切組83例。分別對(duì)產(chǎn)婦分娩前、產(chǎn)后6周、產(chǎn)后3個(gè)月、產(chǎn)后5個(gè)月的盆底肌肉肌電值、Ⅰ類肌肌力與Ⅱ類肌肌力進(jìn)行測(cè)定。結(jié)果剖宮產(chǎn)組與陰道分娩組產(chǎn)后6周及3、5個(gè)月比較,除產(chǎn)后6周兩組肌電位活力值間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)外,其余各時(shí)間點(diǎn)各組肌電值相比較,剖宮產(chǎn)組均明顯高于陰道分娩組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。陰道分娩組中,會(huì)陰側(cè)切組與非側(cè)切組產(chǎn)后6周及3、5個(gè)月比較,兩組間各項(xiàng)肌電值差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。而在產(chǎn)后6周及3、5個(gè)月3次復(fù)查中,剖宮產(chǎn)組Ⅰ類肌肌力分別下降32.26%、35.48%、27.42%,Ⅱ類肌肌力分別下降61.29%、54.84%、46.77%;陰道分娩組Ⅰ類肌肌力分別下降41.79%、52.24%、46.27%;Ⅱ類肌肌力分別下降71.64%、70.15%、62.69%。會(huì)陰側(cè)切組Ⅰ類肌肌力分別下降41.17%、52.94%、44.12%;Ⅱ類肌肌力分別下降79.41%、76.47%、52.94%;非側(cè)切組Ⅰ類肌肌力分別下降42.42%、51.52%、39.39%,Ⅱ類肌肌力分別下降63.64%、63.64%、51.52%。無(wú)論剖宮產(chǎn)組與陰道分娩組,還是會(huì)陰側(cè)切組與非側(cè)切組,盆底Ⅰ類、Ⅱ類肌肌力下降發(fā)生率在組間比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論剖宮產(chǎn)對(duì)早期肌電值的影響較陰道分娩小,對(duì)盆底肌力的影響與陰道分娩相比差異不大;會(huì)陰側(cè)切無(wú)法減輕對(duì)產(chǎn)后盆底肌肉功能的影響;盆底肌肉肌電值與肌力在評(píng)價(jià)產(chǎn)后早期盆底功能時(shí)可能不具有同步性。
[Abstract]:Objective to investigate the relationship between different delivery modes and the changes of myoelectric value and muscle strength of pelvic floor in early postpartum. Methods from January 2014 to January 2015, 36-week pregnancy was performed at Shengjing Hospital affiliated to China Medical University, and the pelvic floor function was detected in the pelvic floor treatment center. 329 pregnant women who met the criteria of inclusion and exclusion were selected as the study objects. All parturients were divided into selective cesarean section group (n = 162) and vaginal delivery group (n = 167) according to different delivery methods.
