【摘要】：目的探討復(fù)蘇周期第5天(D5)單囊胚移植與雙囊胚移植對年齡35歲、卵巢儲備正常的不育癥患者妊娠結(jié)局的影響。方法回顧性分析2013年1月至2015年4月我中心466例年齡35歲、卵巢儲備正常的不育癥患者復(fù)蘇周期D5囊胚移植的妊娠結(jié)局,根據(jù)D5囊胚移植個數(shù)分為D5單囊胚移植組(207例)與D5雙囊胚移植組(259例),對兩組的妊娠結(jié)局進(jìn)行比較。結(jié)果兩組的生化妊娠率、臨床妊娠率、活產(chǎn)率、異位妊娠率、流產(chǎn)率及單卵雙胎率比較均無顯著性差異(P0.05);雙囊胚移植組的多胎妊娠率顯著高于單囊胚移植組(52.6%vs.2.4%)(P0.001);單囊胚移植組的種植率顯著高于雙囊胚移植組(79.2%vs.57.5%)(P0.001);新生兒基本情況的統(tǒng)計顯示,單囊胚移植組新生兒的胎齡及平均出生體重均顯著大于雙囊胚移植組[分別為(38.6±2.1)周vs.(37.4±2.4)周,(3 282.0±523.4)g vs.(2 937.9±645.5)g](P0.001)。結(jié)論對于年齡35歲、卵巢儲備正常,新鮮周期未移植且為第一次復(fù)蘇周期D5囊胚移植的患者,可以建議行D5單囊胚移植,其能獲得與D5雙囊胚移植相似的活產(chǎn)率,并能顯著降低多胎率、延長孕周并改善新生兒出生體重。
[Abstract]:Objective to investigate the effect of single blastocyst transplantation (D5) and double blastocyst transplantation (D5) on pregnancy outcome in infertile patients aged 35 years with normal ovarian reserve. Methods from January 2013 to April 2015, the pregnancy outcomes of 466 infertile patients aged 35 years with normal ovarian reserve during D5 blastocyst transplantation were retrospectively analyzed. According to the number of D 5 blastocysts, we divided them into two groups: D5 single blastocyst transplantation group (207 cases) and D5 double blastocyst transplantation group (259 cases). The pregnancy outcomes of the two groups were compared. Results the biochemical pregnancy rate, clinical pregnancy rate, live delivery rate, ectopic pregnancy rate of the two groups, There was no significant difference in abortion rate and single egg twin embryo rate (P0.05), the multiple pregnancy rate in double blastocyst transplantation group was significantly higher than that in single blastocyst transplantation group (52.6 vs 2.4%) (P0.001), and the implantation rate in single blastocyst transplantation group was significantly higher than that in double blastocyst transplantation group (79.2 vs. 57.5%) (P0.001). The gestational age and average birth weight of single blastocyst transplantation group were significantly higher than those of double blastocyst transplantation group [(38.6 鹵2.1) weeks vs (37.4 鹵2.4) weeks, (3 282.0 鹵523.4) g vs. (2 937.9 鹵645.5) g] (P0.001). Conclusion D5 single blastocyst transplantation can be recommended for 35 years old patients with normal ovarian reserve, fresh cycle and D5 blastocyst transplantation in the first resuscitation cycle, and the living rate of D5 single blastocyst transplantation is similar to that of D5 double blastocyst transplantation. It can significantly reduce the rate of multiple births, prolong the gestational age and improve the birth weight of the newborn.
