【摘要】：目的分析外周血細(xì)胞因子在胎膜早破(PROM)、新生兒圍產(chǎn)期感染中表達(dá)特點(diǎn)以及臨床意義。方法選取2014年6月-2015年7月在醫(yī)院確診為足月胎膜早破并成功分娩的132例孕婦作為胎膜早破組,選取同期足月正常分娩的130名孕婦作為正常對(duì)照組,孕婦在分娩前、分娩后24h內(nèi)空腹抽取外周靜脈血,新生兒在出生后24h內(nèi)抽取外周靜脈血,應(yīng)用免疫學(xué)方法檢測(cè)血清可溶性白細(xì)胞分化抗原14(SCD14)、降鈣素原(PCT)、C-反應(yīng)蛋白(CRP)、干擾素-γ(IFN-γ)以及白細(xì)胞介素-10(IL-10)水平,同時(shí)統(tǒng)計(jì)新生兒圍產(chǎn)期感染情況,對(duì)比分析正常孕婦與胎膜早破孕婦、圍產(chǎn)期感染新生兒與健康新生兒之間外周血細(xì)胞因子表達(dá)水平的差異性。結(jié)果胎膜早破組分娩后SCD14(46.34±8.35)ng/ml、PCT(52.36±8.23)pg/ml、CRP(4.78±1.24)mg/ml、IFN-γ(75.24±9.23)pg/ml、IL-10(6.59±1.25)pg/ml顯著下降,兩組孕婦外周血清SCD14、PCT、CRP、IFN-γ、IL-10差異有統(tǒng)計(jì)學(xué)意義(P0.05);胎膜早破組新生兒外周SCD14(56.35±8.25)ng/ml、PCT(55.32±10.24)pg/ml、CRP(6.01±1.69)mg/ml、IFN-γ(62.21±9.25)pg/ml、新生兒感染率15.15%顯著高于正常對(duì)照組(P0.05),IL-10(5.26±1.02)pg/ml顯著低于對(duì)照組(P0.05);PROM組感染新生兒外周血SCD14(63.52±9.25)ng/ml、PCT(61.32±1.24)pg/ml、CRP(7.26±1.75)mg/ml、IFN-γ(96.73±11.26)pg/ml顯著高于未感染新生兒,而IL-10(4.03±2.01)pg/ml顯著低于未感染新生兒(P0.05)。結(jié)論胎膜早破孕婦和新生兒圍產(chǎn)期感染患兒的外周血炎性細(xì)胞因子表達(dá)水平顯著升高,積極進(jìn)行外周血炎性因子表達(dá)檢測(cè)有利于早期預(yù)測(cè)胎膜早破和新生兒圍產(chǎn)期感染。
[Abstract]:Objective to analyze the expression and clinical significance of peripheral blood cytokines in premature rupture of membranes (prom) and perinatal infection. Methods 132 pregnant women diagnosed in hospital from June 2014 to July 2015 were selected as the group of premature rupture of membranes and 130 pregnant women in the same period of term normal delivery as normal control group. The peripheral venous blood was drawn within 24 hours after delivery on an empty stomach, and the peripheral vein blood was taken from the newborn within 24 hours after birth. The levels of serum soluble leukocyte differentiation antigen 14 (SCD14), procalcitonin (PCT) C-reactive protein (CRP), interferon- 緯 (IFN- 緯) and interleukin-10 (IL-10) were detected by immunological method. The levels of cytokines in peripheral blood of normal pregnant women, premature rupture of membranes, perinatal infection newborns and healthy neonates were compared and analyzed.
