【摘要】：目的探討婦科腫瘤術后泌尿系感染的危險因素、病原菌及耐藥性,為臨床預防及治療婦科腫瘤術后泌尿系感染提供依據。方法選取2014年3月-2016年12月醫(yī)院婦科腫瘤患者912例,收集患者年齡、腫瘤類型等臨床資料,觀察分析泌尿系感染病原學檢測結果與藥物敏感性檢測結果等。結果婦科腫瘤患者術后泌尿系感染120例,感染率為13.16%;多因素logistic回歸分析結果顯示,年齡、術后住院時間、合并糖尿病、留置導尿時間為術后泌尿系感染的影響因素(P0.05);120例術后泌尿系感染患者共培養(yǎng)出163株病原菌,其中革蘭陽性菌44株占26.99%,革蘭陰性菌104株占63.80%,真菌15株占9.20%;大腸埃希菌對氨芐西林、頭孢噻肟、頭孢他啶、頭孢吡肟的耐藥率較高,腸球菌屬對磺胺甲VA唑/甲氧芐啶、阿莫西林/克拉維酸鉀、紅霉素、四環(huán)素、慶大霉素的耐藥率較高。結論婦科腫瘤術后泌尿系感染發(fā)生與高齡、手術時間長、術后住院時間長、合并糖尿病及留置導尿時間長相關;感染以革蘭陰性菌為主;細菌對氨芐西林、頭孢類耐藥率較高,對亞胺培南、美羅培南耐藥率較低。
[Abstract]:Objective to explore the risk factors, pathogens and drug resistance of urinary tract infection after gynecological tumor operation, and to provide evidence for clinical prevention and treatment of urinary tract infection after gynecological tumor operation. Methods from March 2014 to December 2016, 912 patients with gynecological tumors were collected and their age, tumor type and other clinical data were collected, and the results of etiology and drug sensitivity of urinary tract infection were observed and analyzed. Results 120 cases of postoperative urinary tract infection were found in patients with gynecological neoplasms, the infection rate was 13.16.The results of multivariate logistic regression analysis showed that age, postoperative hospitalization time, diabetes mellitus, The time of indwelling catheterization was the influencing factor of urinary tract infection after operation (P0.05) 163 strains of pathogenic bacteria were cultured in 120 cases of postoperative urinary tract infection, among which 44 strains of Gram-positive bacteria accounted for 26.99%, 104 strains of Gram-negative bacteria accounted for 63.80 strains, 15 strains of fungi accounted for 9.20%, Escherichia coli for ampicillin, The resistant rates of cefotaxime, ceftazidime and cefepime were higher. The resistance rates of Enterococcus to sulfamethazol / trimethoprim, amoxicillin / clavulanate potassium, erythromycin, tetracycline and gentamicin were higher. Conclusion the incidence of urinary tract infection after operation of gynecological tumor is related to the age, the operation time is long, the hospital stay time is long, the complications of diabetes mellitus and indwelling urethral catheterization are long, the infection is mainly Gram-negative bacteria, the rate of bacterial drug resistance to ampicillin and cefazolin is higher. The resistance to imipenem and meropenem was low.
