【摘要】：目的系統(tǒng)評(píng)價(jià)n-3多不飽和脂肪酸對(duì)妊娠糖尿病患者胰島功能、血脂代謝和妊娠結(jié)局的影響。方法檢索英文數(shù)據(jù)庫(kù)Medline、EMbase、Cochrane對(duì)照試驗(yàn)中心注冊(cè)數(shù)據(jù)庫(kù)(Cochrance Central register of controlled trials,CENTRAL),檢索語言為英語,檢索起止時(shí)間為2000年1月—2017年3月。檢索詞為gestational diabetes,n-3 polyunsaturated fatty acid,clinical trial。中文數(shù)據(jù)庫(kù)為中國(guó)知網(wǎng),檢索起止時(shí)間為2000年1月—2017年3月,檢索詞為妊娠糖尿病、n-3多不飽和脂肪酸、臨床研究。納入對(duì)妊娠糖尿病患者添加n-3多不飽和脂肪酸的隨機(jī)對(duì)照試驗(yàn),采用Jadad量表進(jìn)行研究質(zhì)量評(píng)價(jià),使用Rev Man5.2軟件進(jìn)行Meta分析。結(jié)果最終納入5篇英文文獻(xiàn),來源于2個(gè)不同國(guó)家的高質(zhì)量研究。n-3組的胰島素釋放(I~2=92%,95%CI=-4.95~-1.03,Z=2.99,P=0.003),新生兒高膽紅素血癥的發(fā)病率(I~2=0%,95%CI=0.07~0.58,Z=2.94,P=0.003),n-3組新生兒住院率低于對(duì)照組(I~2=0%,95%CI=0.07~0.64,Z=2.78,P=0.005),n-3組的血清總膽固醇高于對(duì)照組(I~2=82%,95%CI=2.39~10.61,Z=3.1,P=0.002),差異均有統(tǒng)計(jì)學(xué)意義。空腹血糖(FPG)(I~2=100%,95%CI=-9.87~16.98,Z=0.52,P=0.6)、血清甘油三酯(I~2=99%,95%CI=-40.69~20.85,Z=0.63,P=0.53)、需要接受胰島素治療的比例(I~2=0%,95%CI=0.05~2.16,Z=1.16,P=0.25)和巨大兒比例(I~2=9%,95%CI=0.11~1.45,Z=1.38,P=0.17)組間差異均無統(tǒng)計(jì)學(xué)意義。結(jié)論現(xiàn)有臨床證據(jù)不足以證明n-3脂肪酸對(duì)改善妊娠糖尿病患者胰島素抵抗和母代妊娠結(jié)局有益,但對(duì)新生兒健康狀況有益作用。
[Abstract]:Objective to evaluate the effects of n-3 polyunsaturated fatty acids on islet function, lipid metabolism and pregnancy outcome in patients with gestational diabetes mellitus. Methods the English database Medlinea was retrieved from January 2000 to March 2017 in English as the search language for the Cochrance Central register of controlled trialscentral. The key word is gestational diabetes n-3 polyunsaturated fatty acidical trial. The Chinese database is Chinese information network. The search time is from January 2000 to March 2017. The key term is gestational diabetes mellitus (PDM) and polyunsaturated fatty acid (PUFA). A randomized controlled trial of adding n-3 polyunsaturated fatty acids to gestational diabetes mellitus patients was conducted.
