【摘要】：目的探討妊娠期婦女B族鏈球菌(GBS)感染對(duì)新生兒危害及母兒預(yù)后的影響。方法選取2014年6月至2016年7月曲靖市第一人民醫(yī)院產(chǎn)科門診接受產(chǎn)檢的35~37周3000例待產(chǎn)孕婦陰道和直腸分泌物及分娩后新生兒的臍帶血。分別采用實(shí)時(shí)熒光定量PCR(QF-PCR)法和細(xì)菌培養(yǎng)法對(duì)分泌物及新生兒臍帶血進(jìn)行GBS檢測(cè)。分析GBS感染對(duì)圍產(chǎn)期母兒預(yù)后的影響。結(jié)果 GBS在陰道及直腸的總定植率為10.3%,陰道和直腸GBS定植率分別為9.23%、10.13%,檢出率分別為89.6%、98.4%,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。GBS對(duì)青霉素類、頭孢吡肟、萬古霉素和氨芐西林的敏感度為100%。GBS陽性組與GBS陰性組在母兒結(jié)局如早產(chǎn)、剖宮產(chǎn)、胎膜早破、霉菌性陰道炎、宮內(nèi)感染、胎兒窘迫、新生兒窒息、新生兒肺炎的發(fā)生率比較差異有統(tǒng)計(jì)學(xué)意義(χ2值分別為13.904、4.868、8.382、39.799、46.570、43.624、40.429、91.571,P0.05)。結(jié)論妊娠期婦女GBS感染可能會(huì)對(duì)母兒預(yù)后造成不良影響,臨床上有必要進(jìn)行常規(guī)GBS檢測(cè),并對(duì)GBS陽性菌株進(jìn)行藥敏試驗(yàn),以便有效的進(jìn)行干預(yù)治療,以保證圍產(chǎn)期母兒的健康。
[Abstract]:Objective to investigate the effects of group B streptococcus B (GBS) infection on neonatal hazards and maternal and fetal prognosis in pregnant women. Methods from June 2014 to July 2016, 3000 expectant pregnant women with vaginal and rectal secretions and umbilical cord blood were collected from the obstetrical outpatient clinic of the first people's Hospital of Qujing City from June 2014 to July 2016. The secretion and umbilical cord blood of newborn were detected by real-time fluorescence quantitative PCRF-PCRand bacterial culture. To analyze the effect of GBS infection on perinatal maternal and fetal prognosis. Results the total colonization rate of GBS in vagina and rectum was 10.3, and the colonization rate of GBS in vagina and rectum was 9.23 and 10.13.The positive rate was 89.6and 98.4, respectively. There was no significant difference in penicillin and cefepime between GBS and GBS. The sensitivity of vancomycin and ampicillin was 100. GBS positive group and GBS negative group in maternal and fetal outcomes such as premature delivery, cesarean section, premature rupture of membranes, fungal vaginitis, intrauterine infection, fetal distress, neonatal asphyxia, There was significant difference in the incidence of neonatal pneumonia (蠂 ~ 2 = 13.9044.868 / 8.382U 46.570g / 43.624 / 40.429 / 91.571/ P 0.05 respectively). Conclusion GBS infection in pregnant women may have adverse effects on the prognosis of mother and fetus. It is necessary to carry out routine GBS detection and drug sensitivity test for GBS positive strains in order to effectively intervene and to ensure the health of perinatal mothers and infants.
