【摘要】：目的:對(duì)剖宮產(chǎn)瘢痕妊娠(CSP)保守治療的相關(guān)危險(xiǎn)因素進(jìn)行系統(tǒng)評(píng)價(jià)。方法:計(jì)算機(jī)檢索Pubmed、Embase、Google、中國(guó)知網(wǎng)、維普、萬(wàn)方數(shù)據(jù)庫(kù),并輔以手工檢索,檢索時(shí)限2000~2013年,納入研究的CSP為保守治療失敗的患者,其相關(guān)危險(xiǎn)因素包括超聲檢查分型、胎血管搏動(dòng)、治療前血β-HCG值、年齡、孕周。采用Rev Man 5.0軟件對(duì)納入文獻(xiàn)的數(shù)據(jù)進(jìn)行Meta分析,計(jì)算相關(guān)危險(xiǎn)因素的比值比(OR)及95%可信區(qū)間(CI)。結(jié)果:共篩選出相關(guān)文獻(xiàn)8篇,中文文獻(xiàn)2篇,英文文獻(xiàn)6篇。保守治療患者254例,失敗82例,全部入選患者保守治療失敗率32.28%。Meta分析結(jié)果顯示:包塊型CSP(Z=3.46,OR=0.19,95%CI 0.07~0.48,P=0.0005)為保守治療失敗的危險(xiǎn)因素,而高齡孕產(chǎn)婦、治療前可探及胎血管搏動(dòng)、血β-HCG值20000 U/L及孕周≥8周為CSP保守治療失敗的不確定因素(P0.05)。結(jié)論:包塊型CSP顯著增加治療過(guò)程中嚴(yán)重并發(fā)癥的發(fā)生而導(dǎo)致保守治療失敗,建議對(duì)CSP患者治療前行超聲檢測(cè)評(píng)估分型,以避免嚴(yán)重并發(fā)癥的發(fā)生。
[Abstract]:Objective: to evaluate the risk factors associated with conservative treatment of CSP in cesarean scar pregnancy. Methods: Pubmedmedan Embase Google, China knowledge Network, Weip, Wanfang database were searched by computer, supplemented by manual retrieval, the search time was from 2000 to 2013. CSP was included in the study as the patients who failed conservative treatment. The related risk factors included ultrasonic examination and fetal vascular pulsatility.
