【摘要】：目的分析胎兒上腹部大血管及臟器相對位置改變與異構畸形的關系,探討上腹部橫切面在產(chǎn)前超聲診斷心房異構綜合征中的價值。方法對2013年3月~2016年9月在徐州市婦幼保健院進行胎兒超聲心動圖檢查中診斷為心房異構綜合征病例的胎兒的上腹部橫切面聲像特點進行回顧性分析,分析內(nèi)容包括腹部大血管的位置及其相對關系、胃泡位置、膽囊位置、肝臟的形態(tài)及門靜脈竇的特點。結果 23例胎兒心房異構胎兒均存在上腹部橫切面的聲像改變,其中9例左房異構綜合征均合并腎上段下腔靜脈離斷,奇靜脈或半奇靜脈與腹主動脈并列(占左房異構的100%);14例右房異構均表現(xiàn)為下腔靜脈與腹主動脈并列位于脊柱同側(占右房異構的100%)。22例存在中位肝并出現(xiàn)胃泡和膽囊位置改變(占心房異構的95.7%);1例左房異構胃泡膽囊位置正常(占心房異構的4.3%,占左房異構的11.1%)。結論胎兒上腹部橫切面聲像改變,尤其是上腹部大血管排列關系改變,對提示胎兒心房異構綜合征具有重要價值。
[Abstract]:Objective to analyze the relationship between the relative position of fetal epigastric great vessels and organs and isomerism, and to explore the value of upper abdominal transverse section in prenatal diagnosis of atrial isomerism syndrome. Methods from March 2013 to September 2016 in Xuzhou Maternal and Child Health Hospital, the characteristics of upper abdominal cross-sectional sonography of fetus diagnosed as atrial isomerism syndrome were analyzed retrospectively. The analysis includes the location and relative relationship of the great abdominal vessels, the location of the stomach vesicle, the location of the gallbladder, the morphology of the liver and the characteristics of the portal sinus. Results all of the 23 fetuses with fetal atrial isomerism had transverse-sectional changes in the upper abdomen. Among them, 9 cases of left atrial isomerism syndrome were accompanied by superior segmental inferior vena cava (IVC). Azygos vein or half azygos vein juxtaposed with abdominal aorta (100% of left atrium isomerism 14 cases of right atrial isomerism all show inferior vena cava and abdominal aorta juxtaposed on the same side of the spine (100% of right atrial isomerism. 22 cases have median liver with stomach bubble and bile duct) The change of cystic location (95.7% of atrial isomerism) was normal in 1 case (4.3% of atrial isomerism, 11.1% of left atrial isomerism). Conclusion the changes of the transverse section of fetal upper abdomen, especially the changes of the large vessels arrangement in the upper abdomen, are of great value in indicating fetal atrial isomerism syndrome.
