【摘要】：目的：觀察藥膳方“暖巢煲”合來曲唑治療多囊卵巢綜合征(PCOS)排卵功能障礙性不孕腎虛證的臨床療效,為臨床應(yīng)用提供依據(jù)。 方法：嚴(yán)格按照診斷及納入標(biāo)準(zhǔn),收集2013年1月～2013年8月在湖南中醫(yī)藥大學(xué)第一附屬醫(yī)院婦科門診病人中因PCOS排卵障礙不孕腎虛證的患者60例,遵循隨機(jī)、陽性藥對照的科研設(shè)計方法,分為2組,治療組30例,口服暖巢煲合來曲唑；對照組30例,單純口服來曲唑,連續(xù)治療3個療程,即3個月經(jīng)周期。觀察2組治療前后月經(jīng)情況、中醫(yī)癥候積分、排卵率及妊娠率,安全性指標(biāo)情況及不良反應(yīng)。 結(jié)果：治療組臨床痊愈15例,顯效7例,有效5例,無效3例,總有效率90.00%；對照組臨床痊愈8例,顯效6例,有效9例,無效7例,總有效率76.67%,兩組間臨床療效比較差異有統(tǒng)計學(xué)意義(P0.05)。治療組排卵率為81.18%,對照組排卵率為47.73%(P0.01),差異有統(tǒng)計學(xué)意義,治療組排卵率優(yōu)于對照組。兩組治療后妊娠率比較,治療組妊娠率為33.33%,對照組妊娠率為10.00%(P0.05),差異有統(tǒng)計學(xué)意義,兩組均能使受試者月經(jīng)不調(diào)情況改善及臨床癥狀改善,且兩組間月經(jīng)改善情況比較有差異(P0.05),即治療組療效優(yōu)于對照組。治療后兩組間中醫(yī)癥候積分比較有統(tǒng)計學(xué)意義(P0.05),說明在改善中醫(yī)癥候積分方面治療組療效明顯高于對照組。 結(jié)論：暖巢煲聯(lián)合來曲唑治療PCOS排卵障礙性不孕腎虛證可有效改善患者月經(jīng)及中醫(yī)癥候情況,提高PCOS患者的排卵率及臨床妊娠率�？蔀镻COS排卵功能障礙不孕的婦女提供較好的治療方案。
[Abstract]:Objective: to observe the clinical effect of "warm nest-pot" and letrozole in the treatment of ovulation dysfunction syndrome of PCOS, and to provide the basis for clinical application. Methods: according to the criteria of diagnosis and inclusion, 60 patients with kidney deficiency due to PCOS ovulation disorder were collected from January 2013 to August 2013 in the gynecological outpatient department of the first affiliated Hospital of Hunan University of traditional Chinese Medicine. The design method of positive drug control was divided into two groups: treatment group (n = 30) and control group (n = 30). Menstruation, TCM symptom score, ovulation rate and pregnancy rate, safety index and adverse reaction were observed before and after treatment. Results: in the treatment group, 15 cases were cured, 7 cases were effective, 5 cases were effective, 3 cases were ineffective, and the total effective rate was 90.00.The control group was clinically cured in 8 cases, effective in 6 cases, effective in 9 cases, ineffective in 7 cases. The total effective rate was 76.67 and the difference between the two groups was statistically significant (P 0.05). The ovulation rate of the treatment group was 81.18 and that of the control group was 47.730.The difference was statistically significant. The ovulation rate of the treatment group was better than that of the control group. The pregnancy rate was 33.33 in the treatment group and 10.00 in the control group. The difference was statistically significant. Both groups could improve the menstrual disorder and clinical symptoms. There was significant difference between the two groups in the improvement of menstruation, that is, the curative effect of the treatment group was better than that of the control group. After treatment, the scores of TCM symptoms in the two groups were significantly higher than those in the control group (P 0.05), which indicated that the curative effect of the treatment group was significantly higher than that of the control group in improving the TCM symptom score. Conclusion: the treatment of kidney deficiency syndrome with PCOS ovulation disorder can effectively improve menstruation and TCM symptoms, and increase the ovulation rate and clinical pregnancy rate of PCOS patients. It can provide better treatment for infertile women with PCOS ovulation dysfunction.
