【摘要】：目的:從三級預(yù)防角度探討系統(tǒng)預(yù)防和減少產(chǎn)后出血的措施。方法:對我院2008年1月至2012年12月住院分娩的產(chǎn)婦產(chǎn)后出血440例,對照組1200例,通過多元Logistic回歸分析評價各高危因素的相對風(fēng)險值。結(jié)果:妊娠年齡≥35歲(OR=1.8),孕前體重62.8 kg(OR=1.4),孕次3次(OR=1.4),產(chǎn)次2次(OR=4.3),雙胎(OR=2.2),肌瘤剝出史(OR=4.0),這些因素可以于孕前進行一級預(yù)防。作為二級預(yù)防孕期干預(yù)的高危因素有子癇前期及妊娠期高血壓(OR值為2.5和1.9)、胎兒各生長徑線代表的胎兒預(yù)估體重、硫酸鎂的應(yīng)用史(OR=2.8)、產(chǎn)前Hb108g/L(OR=1.2);產(chǎn)時干預(yù)的高危因素為會陰側(cè)切(OR=1.4)、產(chǎn)道裂傷(OR=1.1)、剖宮產(chǎn)(OR=1.2)、第三產(chǎn)程8分鐘(OR=1.1)。結(jié)論:減少產(chǎn)后出血高危因素,需要通過孕前孕期產(chǎn)時三級預(yù)防完整模式進行綜合干預(yù)。
