【摘要】：目的探討超激光穴位照射聯(lián)合盆底康復(fù)治療絕經(jīng)后壓力性尿失禁對盆底肌力和尿動力學(xué)的影響。方法選取絕經(jīng)后壓力性尿失禁患者66例為研究對象,按信封法隨機分為聯(lián)合組(n=33)和康復(fù)組(n=33)�？祻�(fù)組采用盆底康復(fù)治療儀進行治療,聯(lián)合組在對照組的基礎(chǔ)上采用超激光穴位照射治療,治療周期4周。檢測比較兩組療效、治療前后盆底肌力評分、尿墊試驗評分、尿動力學(xué)及不良反應(yīng)和并發(fā)癥發(fā)生率等。結(jié)果聯(lián)合組治療有效率為87.88%(29/33),高于康復(fù)組的57.58%(19/33),差異有統(tǒng)計學(xué)意義(P0.05)。與治療前比較,聯(lián)合組和康復(fù)組治療2周和4周的尿墊試驗評分降低,盆底肌力評分、控制面積、最大逼尿肌壓力和最大尿道關(guān)閉壓升高(P0.05)。與康復(fù)組比較,聯(lián)合組治療2周和4周的尿墊試驗評分降低,盆底肌力評分、控制面積、最大逼尿肌壓力和最大尿道關(guān)閉壓升高,差異有統(tǒng)計學(xué)意義(P0.05)。兩組不良反應(yīng)和并發(fā)癥發(fā)生率比較差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論超激光穴位照射聯(lián)合盆底康復(fù)治療絕經(jīng)后壓力性尿失禁可有效改善盆底肌力和尿動力學(xué),且其安全性良好,值得臨床推廣應(yīng)用。
[Abstract]:Objective to investigate the effect of hyperlaser acupoint irradiation combined with pelvic floor rehabilitation on pelvic floor muscle strength and urodynamics in postmenopausal stress urinary incontinence. Methods 66 patients with postmenopausal stress urinary incontinence were randomly divided into two groups according to the envelope method: the combined group (n = 33) and the rehabilitation group (n = 33). The rehabilitation group was treated with pelvic floor rehabilitation therapy instrument, and the combined group was treated with super laser acupoint irradiation on the basis of the control group. The treatment period was 4 weeks. The efficacy, pelvic floor muscle strength score, urinary pad test, urodynamics, adverse reactions and complications were measured and compared between the two groups before and after treatment. Results the effective rate of treatment in the combined group was 87.88 / 29 / 33, which was higher than that in the rehabilitation group (57.58 / 33 / 33), and the difference was statistically significant (P 0.05). Compared with before treatment, the scores of urinary pad test in the combined group and rehabilitation group were decreased at 2 and 4 weeks, the pelvic floor muscle strength score, the controlled area, the maximum detrusor pressure and the maximum urethral closure pressure were increased (P 0.05). Compared with the rehabilitation group, the score of urinary pad test decreased, the pelvic floor muscle strength score, the control area, the maximum detrusor pressure and the maximum urethral closure pressure increased in the combined group at 2 and 4 weeks after treatment, and the difference was statistically significant (P 0.05). There was no significant difference in the incidence of adverse reactions and complications between the two groups (P 0.05). Conclusion Ultra-laser acupoint irradiation combined with pelvic floor rehabilitation for postmenopausal stress urinary incontinence can effectively improve pelvic floor muscle strength and urodynamics, and is safe and worthy of clinical application.
