脈沖多普勒取樣容積位置對(duì)胎兒肺動(dòng)脈血流頻譜參數(shù)影響的研究

【摘要】:目的通過(guò)將脈沖多普勒取樣容積(SV)放置在胎兒肺動(dòng)脈瓣口三個(gè)不同位置(a:內(nèi)側(cè),靠近主動(dòng)脈側(cè);b:肺動(dòng)脈瓣口中間;c:外側(cè),遠(yuǎn)離主動(dòng)脈側(cè))分別獲得胎兒肺動(dòng)脈血流頻譜,探討不同的SV位置對(duì)胎兒肺動(dòng)脈血流頻譜參數(shù)的影響,從而為更準(zhǔn)確的測(cè)量提供參考依據(jù)。方法選取424例正常胎兒,根據(jù)不同孕周分成5組,20~23+6周,24~27+6周,28~31+6周,32~35+6周,36~40+6周,分別測(cè)量其肺動(dòng)脈瓣口三個(gè)位置(a、b、c)的速度時(shí)間積分(VTI)、最大血流速度(Vmax)、平均血流速度(Vmean)、最大壓差(PGmax)、平均壓差(PGmean)、射血時(shí)間(ET)、加速時(shí)間(AT)、加速時(shí)間與射血時(shí)間的比值(AT/ET)。不同孕周間VTI、Vmax、Vmean、PGmax、PGmean、ET、AT、AT/ET組間比較采用方差分析,各參數(shù)與孕周的關(guān)系進(jìn)行Pearson相關(guān)分析,所有孕周不同取樣容積位置(a、b、c)的比較采用配對(duì)t檢驗(yàn)。結(jié)果不同孕周間VTI、Vmax、Vmean、PGmax、PGmean、AT、AT/ET組間比較差異均有統(tǒng)計(jì)學(xué)意義(P=0.000),且均與孕周呈線性相關(guān),a組與b組、b組與c組比較AT、AT/ET、Vmax、Vmean有統(tǒng)計(jì)學(xué)意義(P=0.000),a組與c組比較AT、AT/ET、Vmax、Vmean無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 SV位置的不同會(huì)對(duì)胎兒肺動(dòng)脈血流頻譜參數(shù)產(chǎn)生影響。
[Abstract]:Objective to obtain the flow spectrum of fetal pulmonary artery by placing SVV in three different positions of fetal pulmonary valve orifice: medial side, near the aortic side, central part of pulmonary valve, lateral side, far away from the aorta. To study the influence of different SV positions on the parameters of fetal pulmonary artery flow spectrum, and to provide reference for more accurate measurement. Methods 424 normal fetuses were divided into 5 groups according to different gestational weeks. The velocities and time integrals of the three locations of pulmonary valve orifice were measured respectively: VTI, Vmax, Vmean, PGmax, PGmeann, et, ATP, ATP / ETT, and the ratio of the acceleration time to the ejection time, to the ratio of the acceleration time to the ejection time. The variance analysis was used in the comparison of VTII VMEP VMEP VMEP Vmean PGVMEP EET ATP et group during different gestational weeks. The relationship between each parameter and gestational week was analyzed by Pearson correlation analysis. The comparison of all gestational weeks with different sampling volume locations was carried out by paired t test (P < 0.05), and the correlation between each parameter and gestational age was analyzed by Pearson correlation analysis and paired t test was used for the comparison of each parameter. Results during different gestational weeks, there were significant differences in VTII Vmean Vmean PGmaxPmax and PGmean-ATN ATP / et groups, and there was no significant difference in Vmean between group A and group B and group b and group c in comparison with group C and group C. There was no significant difference in Vmean between group A and group C. there was no significant difference between group A and group C in the Vmean value of ATT et P / P P = 0. 05% and P 0. 05%, P 0. 05%, P 0. 05%, P 0. 05% and P 0. 05% respectively, compared with group C and group C. Conclusion the difference of SV position may affect the parameters of fetal pulmonary artery flow spectrum.
