【摘要】：目的:分析探討黃體期添加雌激素對(duì)不明原因不孕患者的影響,為改進(jìn)臨床方案提供一定的理論依據(jù)。方法:選取2011年1月至2015年1月于我院就診的不明原因不孕患者41例與正常生育婦女60例為研究對(duì)象,對(duì)比分析各組雌、孕激素水平及比例分布差異。將不孕組雌激素低下患者分為對(duì)照組和實(shí)驗(yàn)組,兩組均給予孕激素(P)治療,實(shí)驗(yàn)組在此基礎(chǔ)上于黃體期加用雌激素(E2),對(duì)比分析治療效果的改善情況。結(jié)果:不孕組患者排卵后第7d的P、E2及P/E2比值明顯低于正常組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P0.05),而排卵后第3d各組指標(biāo)無(wú)統(tǒng)計(jì)學(xué)差異(P0.05);在不孕患者中,加用E2、P治療的實(shí)驗(yàn)組,排卵后第7d治療組患者的P及P/E2比值改善情況明顯優(yōu)于單用P的對(duì)照組(P0.05),排卵后第3d各組指標(biāo)比較不具有統(tǒng)計(jì)學(xué)差異(P0.05);實(shí)驗(yàn)組妊娠成功率分別明顯高于對(duì)照組(P0.05)。結(jié)論:黃體期添加雌激素可提高不明原因不孕患者的妊娠成功率,為臨床上不明原因不孕患者的預(yù)防和治療提供新思路,具有進(jìn)一步擴(kuò)大研究的價(jià)值。
[Abstract]:Objective: to analyze the effect of estrogen addition in luteal phase on patients with unexplained infertility, and to provide theoretical basis for improving clinical regimen. Methods: from January 2011 to January 2015, 41 infertile patients with unknown causes and 60 normal fertile women were selected as study subjects, and the distribution of estrogen and progesterone levels in each group were compared and analyzed. The infertile patients were divided into two groups: control group and experimental group. Both groups were treated with progesterone (P). The experimental group was treated with estradiol in luteal phase, and the improvement of the therapeutic effect was analyzed. Results: the ratio of E _ 2 and P/E2 in infertile group was significantly lower than that in normal group on the 7th day after ovulation. The difference between the two groups was statistically significant (P 0.05), but there was no significant difference in the indexes of each group on the 3rd day after ovulation. On the 7th day after ovulation, the improvement of P and P/E2 ratio in the treatment group was significantly better than that in the control group treated with P alone (P 0.05), but there was no significant difference between the three groups on the 3rd day after ovulation, and the success rate of pregnancy in the experimental group was significantly higher than that in the control group (P 0.05). Conclusion: estrogen supplementation in luteal phase can increase the pregnancy success rate of patients with unexplained infertility and provide a new idea for the prevention and treatment of unexplained infertility.
