SPARC、E-cadherin、EZH2在子宮內(nèi)膜樣腺癌中的蛋白表達(dá)及臨床意義

【摘要】:目的: 檢測SPARC、E-cadherin及EZH2在增殖期子宮內(nèi)膜(proliferative phase endometrium, PPE)、子宮內(nèi)膜單純性增生(simple endometrial hyperplasia, SEH)、子宮內(nèi)膜復(fù)雜性增生(complex endometrial hyperplasia,CEH)、子宮內(nèi)膜非典型增生(atypical endometrial hyperplasia,AEH)及子宮內(nèi)膜樣腺癌(endometrioid adenocarcinoma, EA)組織中的蛋白表達(dá),探討三種蛋白表達(dá)與EA臨床病理因素之間的關(guān)系及其在EA發(fā)生、發(fā)展中的作用。 方法: 采用免疫組化S-P法檢測PPE、SEH、CEH、AEH及EA組織中SPARC、 E-cadherin、EZH2的表達(dá)情況。 結(jié)果: 1、SPARC、E-cadherin在EA組織中的陽性表達(dá)率分別是29.23%和27.69%,明顯低于PPE、SEH、CEH、AEH組織中二者的表達(dá),差異具有統(tǒng)計(jì)學(xué)意義(P0.01);而EZH2在EA組織中的陽性表達(dá)率為73.85%,明顯高于PPE、SEH、 CEH、AEH的組織中的陽性表達(dá),差異具有統(tǒng)計(jì)學(xué)意義(P0.01)。 2、在EA組織中,SPARC、E-cadherin的陽性表達(dá)與腫瘤的組織病理分級、肌層浸潤深度及淋巴結(jié)轉(zhuǎn)移有關(guān),差異具有統(tǒng)計(jì)學(xué)意義(P0.05);而EZH2的陽性表達(dá)與腫瘤的組織病理分級、肌層浸潤深度有關(guān),差異具有統(tǒng)計(jì)學(xué)意義(P0.05),而與淋巴結(jié)轉(zhuǎn)移無關(guān)(P0.05)。 3、在EA組織中,SPARC和E-cadherin的表達(dá)呈正相關(guān)(rs=0.226,P0.05);SPARC和EZH2的表達(dá)呈負(fù)相關(guān)(rs=-0.201,P0.05);E-cadherin和EZH2的表達(dá)呈負(fù)相關(guān)(rs=-0.214,P0.01)。 結(jié)論: 1、從PPE、SEH、CEH、AEH到EA組織,SPARC、E-cadherin的蛋白表達(dá)呈遞減趨勢,而EZH2的表達(dá)呈遞增趨勢,提示三者不同表達(dá)可能與EA的癌變發(fā)生有關(guān)。 2、SPARC、E-cadherin、EZH2表達(dá)與EA的病理分級、肌層浸潤密切相關(guān);SPARC、E-cadherin表達(dá)還與淋巴結(jié)轉(zhuǎn)移密切相關(guān)。提示三者不同表達(dá)可能在EA的發(fā)展、浸潤和轉(zhuǎn)移等過程中起重要作用。 3、SPARC、E-cadherin及EZH2在EA中的表達(dá)呈現(xiàn)相關(guān)性,提示三種蛋白在EA的發(fā)生、發(fā)展中可能存在相互作用,聯(lián)合檢測SPARC、E-cadherin及EZH2將有助于EA的臨床診斷、治療及療效評價(jià)。
[Abstract]:Objective: To detect the expression of SPARCn E-cadherin and EZH2 in proliferative phase endometrium, PPEI, simple endometrial hyperplasia, SEH, endometrial hyperplasia CEH, atypical endometrial hyperplasia (AEH) and endometrioid adenocarcinoma (EAA) in proliferative endometrium and endometrial carcinoma, the expression of SPARCU E-cadherin and EZH2 in proliferative phase endometrium (PPE), simple endometrial hyperplasia (EZH2), endometrial complex hyperplasia (endometrial), atypical endometrial hyperplasia (AEH) and endometrial adenocarcinoma (EAA) were detected. To explore the relationship between the expression of three proteins and the clinicopathological factors of EA and its role in the development and development of EA. Methods: Immunohistochemical S-P method was used to detect the expression of SPARC, E-cadherin EZH _ 2 in the tissues of PPE-SEHH-CEHH and EA. Results: (1) the positive expression rates of E-cadherin in EA were 29.23% and 27.69%, respectively, which were significantly lower than those in PPESEHCEH and AEH, and the difference was statistically significant (P0.01), while the positive rate of EZH2 in EA was 73.85, which was significantly higher than that in PPESEHand CEHAEH. The difference was statistically significant (P 0.01). 2. The positive expression of E-cadherin was related to the histological grade, the depth of myometrial invasion and lymph node metastasis in EA, but the positive expression of EZH2 was related to the histopathological grade and the depth of myometrial invasion. The difference was statistically significant (P 0.05), but not related to lymph node metastasis (P 0.05). 3. There was a positive correlation between the expression of SPARC and E-cadherin in EA tissues. There was a negative correlation between the expression of SPARC and EZH2 in EA tissues. There was a negative correlation between the expression of E-cadherin and EZH2. Conclusion: 1. The protein expression of SPARCU E-cadherin decreased from PPE-SEH CEH to EA, while the expression of EZH2 increased, suggesting that the different expression of AEH might be related to the carcinogenesis of EA. 2the expression of E-cadherin was closely related to the pathological grade of EA, the expression of E-cadherin in muscularis and the expression of E-cadherin in SPARC+ E-cadherin was also closely related to lymph node metastasis. These results suggest that the different expressions may play an important role in the development, infiltration and metastasis of EA. 3The expressions of E-cadherin and EZH2 in EA were correlated, suggesting that there might be interaction between the three proteins in the development and occurrence of EA. The combined detection of E-cadherin and EZH2 will be helpful to the clinical diagnosis, treatment and evaluation of curative effect of EA.
