Caspase-9在子癇前期患者胎盤中的表達(dá)及意義

【摘要】:目的:子癇前期是一種嚴(yán)重威脅母嬰健康的妊娠期特發(fā)性疾病,其發(fā)病機(jī)制至今尚未完全闡明。有研究發(fā)現(xiàn)滋養(yǎng)細(xì)胞的過度凋亡可能導(dǎo)致子癇前期的發(fā)生,,而caspase家族是重要的凋亡蛋白,其中caspase-9作為凋亡的起始環(huán)節(jié)起著關(guān)鍵性的作用,因此本文主要探討caspase-9在子癇前期重度患者胎盤組織中的表達(dá)和意義。 方法:選取2013年4月~2013年12月于山西醫(yī)科大學(xué)第二臨床醫(yī)院婦產(chǎn)科住院分娩的孕婦,其中25例子癇前期重度患者作為研究組,另外選取25例正常晚期妊娠婦女作為正常對(duì)照組,收集患者全部臨床信息和分娩時(shí)胎盤組織。用免疫組織化學(xué)二步法檢測(cè)caspase-9在胎盤組織中的定位及表達(dá),并用BI-2000圖像分析系統(tǒng)進(jìn)行圖處理;western-blot方法檢測(cè)caspase-9在胎盤組織中的定量表達(dá)。 統(tǒng)計(jì)分析采用SPSS16.0軟件,所有數(shù)據(jù)均用均數(shù)±標(biāo)準(zhǔn)差表示。兩組間的臨床數(shù)據(jù)和不同指標(biāo)表達(dá)之間的差異均采用兩獨(dú)立樣本t檢驗(yàn)進(jìn)行評(píng)價(jià),以α=0.05為研究水準(zhǔn),P<0.05為差異有統(tǒng)計(jì)學(xué)意義。 結(jié)果:1.兩組研究資料的比較:研究組與正常對(duì)照組在年齡及孕齡上無顯著性差異(P>0.05);在收縮壓、舒張壓、新生兒體重、新生兒評(píng)分上有顯著性差異(P<0.05)。 2.免疫組化檢測(cè)結(jié)果:研究組和正常對(duì)照組在胎盤滋養(yǎng)細(xì)胞的胞質(zhì)中均可見caspase-9的陽(yáng)性表達(dá),且在子癇前期組胎盤滋養(yǎng)細(xì)胞中caspase-9的表達(dá)(OD:0.322±0.04)顯著高于正常對(duì)照組(OD:0.258±0.05),差異有顯著性(t=-5.227,P<0.01)。 3. Western-blot檢測(cè)結(jié)果:子癇前期組胎盤滋養(yǎng)細(xì)胞中caspase-9的表達(dá)(灰度值:2.58±0.47)高于正常對(duì)照組(灰度值:1.66±0.19),差異有顯著性(t=-3.693,P<0.05)。 結(jié)論:caspase-9表達(dá)于胎盤滋養(yǎng)細(xì)胞的胞質(zhì)中,且在子癇前期重度患者胎盤滋養(yǎng)細(xì)胞的胞質(zhì)中的表達(dá)升高,提示在妊娠婦女胎盤組織中caspase-9作為一種凋亡蛋白,它的表達(dá)水平可能與子癇前期的發(fā)病有關(guān)。
[Abstract]:Objective: preeclampsia is a serious threat to maternal and child health, gestational idiopathic disease, its pathogenesis has not been fully elucidated. Some studies have found that excessive apoptosis of trophoblast may lead to preeclampsia, and caspase family is an important apoptotic protein, in which caspase-9 plays a key role in the initiation of apoptosis. Therefore, we mainly studied the expression and significance of caspase-9 in placenta of severe preeclampsia. Methods: from April 2013 to December 2013, 25 pregnant women with severe preeclampsia and 25 normal women with late pregnancy were selected as the control group, who were given birth in gynecology and obstetrics department of the second Clinical Hospital of Shanxi Medical University from April 2013 to December 2013. Collect all clinical information and placental tissue during delivery. The localization and expression of caspase-9 in placenta were detected by immunohistochemical two-step method, and the quantitative expression of caspase-9 in placenta was detected by BI-2000 image analysis system. SPSS16.0 software is used for statistical analysis, and all data are represented by mean 鹵standard deviation. The differences of clinical data and expression of different indexes between the two groups were evaluated by two independent samples t test, and the difference was statistically significant with 偽 -0.05 as the research level (P < 0.05). The result is 1: 1. There was no significant difference in age and gestational age between the study group and the normal control group (P > 0.05), but there was significant difference in systolic blood pressure, diastolic blood pressure, neonatal weight and neonatal score (P < 0.05). 2. The results of immunohistochemistry: the positive expression of caspase-9 was found in the cytoplasm of placental trophoblast in both study group and normal control group, and the expression of caspase-9 in placental trophoblastic cells in preeclampsia group was significantly higher than that in normal control group (OD0.322 鹵0.04), and the difference was significant (P < 0.01). 3. Results of Western-blot: the expression of caspase-9 in placental trophoblast of preeclampsia group (grayscale: 2.58 鹵0.47) was higher than that of normal control group (grayscale: 1.66 鹵0.19, P < 0.05). Conclusion the results suggest that caspase-9 is a kind of apoptotic protein in placenta of pregnant women, and it is expressed in the cytoplasm of placental trophoblastic cells and in the cytoplasm of placental trophoblast of severe preeclampsia. Its expression may be associated with preeclampsia.
