【摘要】：目的:研究臨床早期子宮內(nèi)膜癌腹膜后淋巴結(jié)轉(zhuǎn)移的相關(guān)因素,為子宮內(nèi)膜癌有選擇實(shí)施腹膜后淋巴結(jié)切除尋找依據(jù)。方法:回顧分析2012年6月至2017年3月于我院行手術(shù)治療的335例臨床早期子宮內(nèi)膜癌患者的術(shù)后病理資料,采用卡方檢驗(yàn)進(jìn)行單因素分析,Logistic回歸進(jìn)行多因素分析。結(jié)果:335例臨床早期子宮內(nèi)膜癌中,24例腹膜后淋巴結(jié)轉(zhuǎn)移,淋巴結(jié)轉(zhuǎn)移率7.16%。單因素分析提示,子宮肌層浸潤(rùn)深度、宮頸管浸潤(rùn)、脈管癌栓與早期子宮內(nèi)膜癌的腹膜后淋巴結(jié)轉(zhuǎn)移有關(guān);Logistic多因素分析提示,宮頸管累及、脈管癌栓是臨床早期子宮內(nèi)膜癌腹膜后淋巴結(jié)轉(zhuǎn)移的獨(dú)立高危因素。結(jié)論:宮頸管浸潤(rùn)和脈管癌栓是早期子宮內(nèi)膜癌腹膜后淋巴結(jié)轉(zhuǎn)移的重要高危因素,是早期子宮內(nèi)膜癌實(shí)施腹膜后淋巴結(jié)切除的重要依據(jù)。
[Abstract]:Objective: to study the related factors of retroperitoneal lymph node metastasis in early stage of endometrial carcinoma, and to find the basis for selective retroperitoneal lymph node resection in endometrial carcinoma. Methods: the postoperative pathological data of 335 patients with early clinical endometrial carcinoma who underwent surgical treatment in our hospital from June 2012 to March 2017 were retrospectively analyzed. Results 24 cases of retroperitoneal lymph node metastasis were found in 33 cases of clinical early endometrial carcinoma, and the lymph node metastasis rate was 7.16%. Univariate analysis showed that the depth of myometrium invasion, cervical canal infiltration, vascular tumor embolus and retroperitoneal lymph node metastasis were associated with early endometrial carcinoma. Vascular tumor embolus is an independent risk factor for retroperitoneal lymph node metastasis in early stage of endometrial carcinoma. Conclusion: cervical canal invasion and vascular tumor embolus are important risk factors for retroperitoneal lymph node metastasis in early endometrial carcinoma and are important evidence for retroperitoneal lymph node resection in early endometrial carcinoma.
