【摘要】：目的系統(tǒng)評(píng)價(jià)目前主要采用的2種瘢痕子宮中期妊娠引產(chǎn)方法的療效。 方法計(jì)算機(jī)檢索MEDLINE、EMbase、萬方、維普和CNKI，檢索時(shí)限均從建庫(kù)至2013年11月，納入關(guān)于瘢痕子宮中期妊娠引產(chǎn)且引產(chǎn)方法包括米非司酮聯(lián)合米索前列醇及米非司酮聯(lián)合依沙吖啶2種方法的隨機(jī)對(duì)照試驗(yàn)，進(jìn)行文獻(xiàn)篩選及質(zhì)量評(píng)價(jià)，采用RevMan5.2軟件進(jìn)行統(tǒng)計(jì)分析。 結(jié)果共納入8篇文獻(xiàn)，文獻(xiàn)質(zhì)量評(píng)價(jià)根據(jù)Jadad量表評(píng)分標(biāo)準(zhǔn)。Meta分析結(jié)果表明，，米非司酮聯(lián)合依沙吖啶組（試驗(yàn)組）與米非司酮聯(lián)合米索前列醇（對(duì)照組）相比，能明顯縮短宮縮發(fā)動(dòng)時(shí)間（P0.00001，WMD=-12.60，95%CI[-17.17,-8.03]）及引產(chǎn)總時(shí)間（P=0.0005，WMD=-10.50，95%CI[-16.46,-4.55]），降低胎盤胎膜殘留率（Z=3.07，P=0.002，95%CI[0.37,0.80]）、增高引產(chǎn)成功率（P=0.02，Z=2.31，95%CI[0.19,0.88]），但2組在總產(chǎn)程（P=0.10，WMD=-2.16，95%CI[-4.73,0.42]）及產(chǎn)后24小時(shí)內(nèi)陰道出血量（P=0.11， WMD=-29.01，95%CI[-64.12,6.09]）差異均無統(tǒng)計(jì)學(xué)意義。 結(jié)論米非司酮聯(lián)合依沙吖啶用于瘢痕子宮中期妊娠引產(chǎn)在宮縮發(fā)動(dòng)時(shí)間、引產(chǎn)總時(shí)間、胎盤胎膜殘留率及引產(chǎn)成功率方面均優(yōu)于米非司酮聯(lián)合米索前列醇，但前者在引產(chǎn)總產(chǎn)程及產(chǎn)后24小時(shí)內(nèi)陰道出血量方面未顯示明顯優(yōu)勢(shì)。
[Abstract]:Objective to evaluate the efficacy of two main methods of inducing labor in the second trimester of cicatricial uterus. Methods MEDLINE EMbase, Wanfang, Weip and CNKI were searched by computer. A randomized controlled trial was conducted on the methods of induced labor and induced labor in the second trimester of cicatricial uterus, including mifepristone combined with misoprostol and mifepristone combined with ethacridine, for literature screening and quality evaluation. RevMan5.2 software was used for statistical analysis. Results A total of 8 articles were included.
