760名女性生殖道高危型人乳頭瘤病毒感染自然轉(zhuǎn)歸規(guī)律及影響因素

【摘要】:目的了解西安地區(qū)女性生殖道高危型人乳頭瘤病毒(HR-HPV)感染自然轉(zhuǎn)歸規(guī)律及影響因素。方法采用HPV-DNA分型檢測(cè)技術(shù)對(duì)760例初次感染HR-HPV的西安地區(qū)女性進(jìn)行隨訪檢測(cè),必要時(shí)行液基細(xì)胞學(xué)和/或陰道鏡檢查。結(jié)果 760名女性生殖道HR-HPV自然清除率為71.58%,中位時(shí)間8.10個(gè)月;持續(xù)感染率為22.63%,中位時(shí)間17.23個(gè)月;進(jìn)展率為5.79%,中位時(shí)間29.34個(gè)月。50歲以上年齡組自然清除率(P=0.000)、持續(xù)感染率(P=0.000)和進(jìn)展率(P=0.040)與其他年齡組相比差異有統(tǒng)計(jì)學(xué)意義。多重感染組持續(xù)感染率明顯低于單一感染組(P=0.010),但其中位時(shí)間明顯長(zhǎng)于單一感染組(P=0.018)。易進(jìn)展型別為HPV-16、HPV-33、HPV-58、HPV-18、HPV-52、HPV-68,其中HPV-16在宮頸上皮內(nèi)瘤變(CIN)Ⅲ/宮頸癌(CC)中最常見,占85.00%。結(jié)論多數(shù)HR-HPV感染在2年內(nèi)被清除,僅少數(shù)進(jìn)展為CIN/CC。50歲以上女性自然清除率低,持續(xù)感染率和進(jìn)展率高,多重感染影響病毒持續(xù),HPV-16是本地區(qū)最常見的致癌株。
[Abstract]:Objective to investigate the natural outcome and influencing factors of high risk human papillomavirus (HR-HPVV) infection in female genital tract in Xi'an. Methods HPV-DNA typing technique was used to detect 760 women in Xi'an area who were first infected with HR-HPV, and liquid-based cytology and / or colposcopy were performed when necessary. Results the natural clearance rate of HR-HPV in 760 female genital tract was 71.58, the median time was 8.10 months, the persistent infection rate was 22.633.The median time was 17.23 months.
