【摘要】：目的探討雙J管置入和輸尿管鏡碎石術(shù)對(duì)于保守治療無(wú)效的妊娠期輸尿管結(jié)石的治療效果。方法回顧性分析中國(guó)醫(yī)科大學(xué)附屬盛京醫(yī)院泌尿外科2007年1月至2015年1月收治的保守治療無(wú)效的38例妊娠期輸尿管結(jié)石患者。根據(jù)治療方法不同分為雙J管組(n=21)和輸尿管鏡組(n=17)。比較2種治療方法在成功率、并發(fā)癥、手術(shù)時(shí)間、治療總費(fèi)用、住院時(shí)間等方面的差異。結(jié)果雙J管組成功17例(81.0%)。輸尿管鏡組成功15例(88.2%)。雙J管組并發(fā)癥發(fā)生率(14.3%)較輸尿管鏡組(11.8%)高。雙J管組在手術(shù)時(shí)間、花費(fèi)及住院時(shí)間方面均較輸尿管鏡組低(P0.05)。成功施行輸尿管鏡手術(shù)的患者無(wú)需進(jìn)一步治療,雙J管組則需要在孕婦分娩后進(jìn)一步處理結(jié)石。結(jié)論輸尿管鏡手術(shù)和置入雙J管均為治療妊娠期輸尿管結(jié)石的有效方法。輸尿管鏡手術(shù)成功率高,可直接處理結(jié)石,而置入雙J管簡(jiǎn)便、快捷,在某些情況具有一定優(yōu)勢(shì)。需根據(jù)患者的不同情況選擇合適的治療方法。
[Abstract]:Objective to evaluate the efficacy of double J tube placement and ureteroscopic lithotripsy in the treatment of ureteral calculi in pregnancy. Methods from January 2007 to January 2015, 38 cases of ureteral calculi with pregnancy were treated in Shengjing Hospital of China Medical University. The patients were divided into double J tube group (n = 21) and ureteroscope group (n = 17). The difference of success rate, complication, operation time, total cost of treatment and hospital stay were compared between the two methods. Results 17 cases of double J tube group were successful. In ureteroscopy group, 15 cases were successful. The incidence of complications in double J tube group was 14. 3% higher than that in ureteroscope group 11. 8%). The operation time, cost and hospitalization time of double J tube group were lower than that of ureteroscopy group (P 0.05). Successful ureteroscopy patients do not need further treatment, and double J tube groups need further management of stones after delivery. Conclusion ureteroscope and double J tube are effective methods for the treatment of ureteral calculi during pregnancy. Ureteroscopy has a high success rate and can be directly treated with stones, and the placement of double J tubes is simple, quick and has some advantages in some cases. The appropriate treatment should be selected according to the patient's different conditions.
