【摘要】：目的 觀察中晚孕期法洛氏四聯(lián)癥胎兒(ToF)大腦溝回發(fā)育的超聲表現(xiàn)和腦血流灌注情況,評(píng)價(jià)ToF胎兒大腦溝回發(fā)育。方法 分析2015年12月~2016年10月在南方醫(yī)科大學(xué)附屬深圳市婦幼保健院超聲醫(yī)學(xué)科診斷為ToF的40名中晚孕期(23周~33周6天)胎兒的大腦溝回資料,評(píng)估大腦溝回指標(biāo):頂枕溝深度、角度及頂枕溝切面半球深度、距狀溝深度及距狀溝切面半球深度、外側(cè)裂寬度及深度、腦島未覆蓋寬度、外側(cè)裂切面半球深度、腦島未覆蓋率、頭圍、雙頂徑;評(píng)估血流動(dòng)力學(xué)指標(biāo):臍動(dòng)脈阻力指數(shù)、臍動(dòng)脈搏動(dòng)指數(shù)、大腦中動(dòng)脈阻力指數(shù)、大腦中動(dòng)脈搏動(dòng)指數(shù)、腦胎盤血流阻力指數(shù)比、腦胎盤血流比,對(duì)上述參數(shù)測(cè)值進(jìn)行統(tǒng)計(jì)分析。結(jié)果 中晚孕期ToF胎兒的頂枕溝深度、外側(cè)裂深度、距狀溝深度、頂枕溝切面半球深度、距狀溝切面半球深度、雙頂徑和頭圍較對(duì)照組小(P0.05);ToF胎兒大腦中動(dòng)脈搏動(dòng)指數(shù)、大腦中動(dòng)脈阻力指數(shù)、腦胎盤血流阻力指數(shù)比和腦胎盤血流比比對(duì)照組小(P0.05)。結(jié)論 中晚孕期ToF胎兒的大腦溝回發(fā)育較正常兒延遲,表現(xiàn)為頂枕溝、外側(cè)裂和距狀溝淺,頂枕溝和腦島閉合延遲,心臟結(jié)構(gòu)異常導(dǎo)致血流動(dòng)力學(xué)改變可能是ToF胎兒大腦溝回發(fā)育遲緩的一個(gè)原因。
[Abstract]:Objective to observe the development of cerebral sulcus gyrus and cerebral blood perfusion in tetralogy of Fallot's fetus during middle and late pregnancy, and to evaluate the development of cerebral sulcus gyrus of ToF fetus. Methods from December 2015 to October 2016, the data of cerebral sulcus gyrus of 40 fetuses diagnosed as ToF in Shenzhen Maternal and Child Health Hospital of Southern Medical University from December 2015 to October 2016 were analyzed. The indexes of sulcus gyrus were as follows: the depth of occipital sulcus, the angle and depth of occipital sulcus, the depth of distance from sulcus, the width and depth of lateral fissure, the width of uncovered cerebral island, the depth of cutting surface of lateral fissure. Assessment of hemodynamic parameters: umbilical artery resistance index, umbilical artery pulsatility index, middle cerebral artery resistance index, middle cerebral artery pulsatility index, cerebral placental flow resistance index ratio, The blood flow ratio of cerebral placenta was statistically analyzed. Results the parietal sulcus depth, lateral fissure depth, distance sulcus depth, hemispheric depth, diparietal diameter and head circumference of ToF fetus during middle and late pregnancy were smaller than those in the control group. The resistance index of the middle cerebral artery, the ratio of cerebral placental blood flow resistance index and the cerebral placental blood flow ratio were lower than those of the control group (P 0.05). Conclusion the development of sulcus gyrus of ToF fetus in middle and late pregnancy is more delayed than that in normal infants, including parietal occipital sulcus, lateral fissure and superficial talus sulcus, and delayed closure of parietal occipital sulcus and cerebral island. Abnormal cardiac structure and hemodynamic changes may be one of the causes of delayed growth of the sulcus gyrus of ToF fetus.
