【摘要】：目的觀察剖宮產(chǎn)患者凝血四項(xiàng)及血小板水平,探討其在產(chǎn)后出血與產(chǎn)后輸血中的意義。方法選擇剖宮產(chǎn)患者242例,根據(jù)產(chǎn)后輸血量分為正常組(未輸血)125例、少量出血組(輸血500~1 600 m L)79例、大量出血組(輸血1 600 m L)74例。采用全自動血凝分析儀、血常規(guī)分析儀檢測3組產(chǎn)前及大量出血組輸血前后凝血四項(xiàng)及血小板水平。結(jié)果正常組、少量出血組、大量出血組產(chǎn)前PT、APTT依次升高,FIB、PLT依次降低,P均0.05。與輸血前比較,大量出血組輸血后第3天PT、APTT、TT降低,FIB、PLT升高,P均0.05。結(jié)論剖宮產(chǎn)患者產(chǎn)前、輸血過程中均出現(xiàn)凝血四項(xiàng)、PLT水平異常,監(jiān)測其變化有助于預(yù)防產(chǎn)后出血及輸血并發(fā)癥發(fā)生。
[Abstract]:Objective to observe the level of blood coagulation and platelet in patients with cesarean section and to explore its significance in postpartum hemorrhage and postpartum blood transfusion. Methods 242 cases of cesarean section were divided into normal group (125 cases without blood transfusion), small amount of bleeding group (79 cases with blood transfusion of 1 600 ml / L) and massive hemorrhage group (74 cases of blood transfusion 1 600 mL / L) according to postpartum blood transfusion volume. The blood coagulation and platelet levels before and after blood transfusion were detected by automatic hemagglutination analyzer and blood routine analyzer. Results in the normal group, the small amount of bleeding group and the large amount of bleeding group, the serum PTT APTT increased in turn and decreased in turn (P 0.05). Compared with that before blood transfusion, PTT APTTTT decreased significantly in patients with massive hemorrhage on the 3rd day after blood transfusion, and increased in FIBPLT (P 0.05). Conclusion the abnormal level of four items of PLT in patients with cesarean section before and after blood transfusion. Monitoring the changes of PLT is helpful to prevent postpartum hemorrhage and complications of blood transfusion.
