【摘要】：目的探討妊娠合并急性闌尾炎的臨床特點(diǎn)和診療策略。方法對2010年1月至2016年6月45例于該院手術(shù)確診為妊娠合并急性闌尾炎病例的臨床資料進(jìn)行回顧性分析。結(jié)果 45例患者均出現(xiàn)腹痛表現(xiàn),其中轉(zhuǎn)移性右下腹痛15例(33.3%),腹部壓痛44例(97.8%)。術(shù)中發(fā)現(xiàn)闌尾壞疽4例,大網(wǎng)膜壞死1例。術(shù)后并發(fā)癥共5例,其中切口愈合不良2例,感染性休克1例,炎性腸梗阻1例,不全性粘連性腸梗阻1例。術(shù)后11例患者(孕22~(+3)~36~(+2)周)出現(xiàn)先兆流產(chǎn)或先兆早產(chǎn),最終流產(chǎn)1例(孕25~(+3)周),早產(chǎn)3例(孕28~(+4)~30~(+4)周)。結(jié)論妊娠合并急性闌尾炎以腹痛為主要臨床表現(xiàn),易出現(xiàn)嚴(yán)重并發(fā)癥。
[Abstract]:Objective to investigate the clinical characteristics and diagnosis and treatment of pregnancy with acute appendicitis. Methods the clinical data of 45 cases of pregnancy with acute appendicitis diagnosed in our hospital from January 2010 to June 2016 were retrospectively analyzed. Results abdominal pain was found in all 45 patients, including metastatic right lower abdominal pain in 15 cases, abdominal tenderness in 44 cases and abdominal tenderness in 44 cases. 4 cases of appendiceal gangrene and 1 case of greater omentum necrosis were found during operation. There were 5 cases of postoperative complications, including 2 cases of poor wound healing, 1 case of septic shock, 1 case of inflammatory intestinal obstruction and 1 case of incomplete adhesive intestinal obstruction. There were 11 cases of threatened abortion or threatened premature delivery in 11 patients (pregnancy 22 weeks), 1 case of final abortion (pregnancy 25 weeks (3) weeks old) and 3 cases of premature delivery (pregnancy 28 cases (4 cases) with 30 cases (4) weeks of gestation). Conclusion the main clinical manifestation of pregnancy with acute appendicitis is abdominal pain, which is prone to severe complications.
