影響體外受精—胚胎移植臨床妊娠結(jié)局的多因素分析

【摘要】:目的探討影響體外受精—胚胎移植(IVF-ET)臨床妊娠結(jié)局的相關(guān)因素。方法回顧性分析2013年1月~2015年12月于本中心行IVF/ICSI-ET治療的鮮胚移植胚胎著床率為100%或0%的594例患者資料,比較(全胚著床)妊娠組與(零胚著床)非妊娠組的基本臨床資料,采用二元Logistic回歸模型分析可能影響妊娠結(jié)局的相關(guān)因素;分別比較不同年齡階段和HCG日子宮內(nèi)膜厚度的助孕結(jié)局。結(jié)果妊娠組和非妊娠組間年齡、不孕年限、HCG日子宮內(nèi)膜厚度、Gn總量、優(yōu)質(zhì)胚胎數(shù)差異均有統(tǒng)計學(xué)意義(P0.05)。進入二元Logistic回歸模型的相關(guān)因素為年齡(OR=-0.118,95%CI=0.845~0.935,P=0.000),優(yōu)質(zhì)胚胎數(shù)(OR=0.081,95%CI=1.016~1.156,P=0.014),HCG日子宮內(nèi)膜厚度(OR=0.203,95%CI=1.088~1.380,P=0.001)。年齡≥38歲組臨床妊娠率顯著低于35歲各組(P0.05);HCG日子宮內(nèi)膜厚度≥8mm各組臨床妊娠率均顯著高于內(nèi)膜厚度8mm組(P0.05)。結(jié)論年齡及優(yōu)質(zhì)胚胎數(shù)、HCG日子宮內(nèi)膜厚度為臨床妊娠結(jié)局的獨立影響因素。年齡≥38歲患者臨床妊娠率顯著降低。HCG日子宮內(nèi)膜厚度≥8mm有助于提高臨床妊娠率。
[Abstract]:Objective to investigate the related factors influencing the clinical pregnancy outcome of IVF-ETS in vitro fertilization-embryo transfer (IVF-et). Methods the data of 594 cases of fresh embryo implantation treated with IVF/ICSI-ET from January 2013 to December 2015 in our center were retrospectively analyzed. The clinical data of pregnant group (full embryo implantation) and non-pregnancy group (zero embryo implantation) were compared. A binary Logistic regression model was used to analyze the related factors that might influence the outcome of pregnancy and to compare the outcome of assisted pregnancy at different ages and on the day of HCG. Results there were significant differences in the total endometrial thickness and the number of high quality embryos between the gestational group and the non-pregnant group, as well as between the infertile years and the day of HCG. There were significant differences in the number of high quality embryos between the two groups (P 0.05).
