【摘要】：目的:評(píng)價(jià)原因不明復(fù)發(fā)性流產(chǎn)(URSA)主動(dòng)免疫治療的臨床效果。方法:2001年2月至2015年2月在我院門診就診的URSA患者1005例為研究對(duì)象,取其丈夫或與女方無血緣關(guān)系的第三方男性的肘靜脈血20 ml,分離淋巴細(xì)胞于孕前后注射于患者前臂,使其產(chǎn)生主動(dòng)免疫,并隨訪妊娠結(jié)局。結(jié)果:1005例URSA患者中共妊娠894例(88.96%),其中足月分娩718例(71.44%),隨訪截止時(shí),尚在妊娠中91例(9.05%),再次自然流產(chǎn)85例(8.46%);無早產(chǎn)病例;未受孕111例(11.04%)。年齡32歲者流產(chǎn)率明顯高于≤32歲者(P0.05)。孕后免疫治療2~3次者比孕后未治療或僅治療1次者的足月分娩率明顯更高(P0.05),流產(chǎn)率明顯更低(P0.05)。結(jié)論:主動(dòng)免疫治療URSA患者療效肯定,妊娠成功率高,年齡大于32歲者妊娠后應(yīng)早期保胎治療;免疫治療后3個(gè)月內(nèi)受孕,確認(rèn)早孕后再免疫治療2~3次效果更好。
[Abstract]:Objective: to evaluate the clinical effect of active immunotherapy for unexplained recurrent abortion. Methods: from February 2001 to February 2015, 1005 URSA patients in our outpatient clinic were studied. The blood samples of the elbow vein of their husband or the third party male who had no blood relationship with the woman were collected and injected into the patient's forearm before pregnancy. Active immunity was produced and pregnancy outcome was followed up. Results A total of 894 pregnancies were found in 894 cases of URSA, including 718 cases of full-term delivery. At the end of follow-up, 91 cases were still in pregnancy, 85 cases had spontaneous abortion, 85 cases had spontaneous abortion, no cases were premature, and 111 cases were not pregnant.
