【摘要】:研究背景及目的:子宮內(nèi)膜異位癥(Endometriosis, EMs)是婦科常見的難治性良性疾病之一,在生育年齡女性的發(fā)病率約為6-10%。腹腔鏡仍是目前診斷子宮內(nèi)膜異位癥的金標(biāo)準(zhǔn)。端粒(telomere)位于真核生物染色體的末端,具有維持染色體穩(wěn)定的功能,其長度對于控制細胞有絲分裂次數(shù)具有重要作用。端粒酶(telomerase)具有維持端粒長度的作用,端粒酶的活化可以使細胞克服衰老獲得無限分裂的能力。端粒酶逆轉(zhuǎn)錄酶(human telomerase reverse transcriptase, hTERT)是端粒酶的催化亞基,對細胞內(nèi)端粒酶的活性起決定性作用。研究證明,hTERT在多種腫瘤細胞中高表達,而hTERT在子宮內(nèi)膜異位癥中的研究較少。本文通過研究hTERT在子宮內(nèi)膜異位癥的在位內(nèi)膜組織及異位內(nèi)膜組織中的表達情況,評估hTERT在子宮內(nèi)膜異位癥的發(fā)病機制中可能起到的作用。 方法:應(yīng)用免疫組織化學(xué)染色sP法檢測54例子宮內(nèi)膜異位癥患者及15例正常子宮內(nèi)膜細胞內(nèi)hTERT的表達情況,比較hTERT在子宮內(nèi)膜異位癥在位內(nèi)膜組織及異位內(nèi)膜組織中的表達與正常子宮內(nèi)膜中hTERT表達的差異,并且分析在子宮內(nèi)膜異位癥中hTERT表達情況與CA125、r-AFS分期及年齡之間的相關(guān)關(guān)系。 結(jié)果:子宮內(nèi)膜異位癥在位內(nèi)膜中hTERT的表達水平比正常子宮內(nèi)膜中hTERT的表達水平顯著增高(p0.05);子宮內(nèi)膜異位癥異位病灶處hTERT的表達水平比在位內(nèi)膜中hTERT的表達水平顯著增高(p0.05),并且比正常子宮內(nèi)膜組織中hTERT的表達水平明顯增高(p0.05)。在子宮內(nèi)膜異位癥在位內(nèi)膜和正常內(nèi)膜中hTERT的表達與內(nèi)膜的周期性變化無明顯差異(p0.05)。hTERT在子宮內(nèi)膜異位癥在位內(nèi)膜在增生期的表達比正常子宮內(nèi)膜增生期hTERT的表達顯著增強(p0.05)。hTERT在子宮內(nèi)膜異位癥在位內(nèi)膜在月經(jīng)分泌期中的表達比正常子宮內(nèi)膜分泌期hTERT的表達無明顯差異(p0.05)。無論是在正常內(nèi)膜還是在子宮內(nèi)膜異位癥的在位內(nèi)膜中hTERT的表達與月經(jīng)周期變化無明顯統(tǒng)計學(xué)差異(p0.05)。子宮內(nèi)膜異位癥中hTERT的表達與血清中CA125水平呈正相關(guān)((p0.05,r0);與r-AFS分期(p0.05)和年齡(p0.05)無明顯相關(guān)性。 結(jié)論:hTERT在子宮內(nèi)膜異位癥中的異常表達說明hTERT在子宮內(nèi)膜異位癥的發(fā)生過程中起一定的作用,子宮內(nèi)膜異位癥的發(fā)生可能與在位內(nèi)膜細胞及異位細胞的異常增生有關(guān)。hTERT的免疫組織化學(xué)染色聯(lián)合血清CA125可對評估子宮內(nèi)膜異位癥的惡變有一定的意義。
[Abstract]:Background and objective: Endometriosism (EMs) is one of the common refractory benign diseases in gynecology. Laparoscopy is still the gold standard for the diagnosis of endometriosis. Telomere, which is located at the end of eukaryotic chromosome, has the function of maintaining chromosome stability, and its length plays an important role in controlling the number of cell mitosis. Telomerase has the function of maintaining telomere length. The activation of telomerase can make cells overcome aging and gain infinite division ability. Human telomerase reverse transcriptase (hTERT) is a catalytic subunit of telomerase, which plays a decisive role in telomerase activity. It has been demonstrated that hTERT is highly expressed in various tumor cells, while hTERT is rarely studied in endometriosis. By studying the expression of hTERT in eutopic endometrium and ectopic endometrium of endometriosis, we evaluated the role of hTERT in the pathogenesis of endometriosis. Methods: the expression of hTERT in 54 cases of endometriosis and 15 cases of normal endometrium were detected by immunohistochemical staining SP method. To compare the expression of hTERT in eutopic endometrium and ectopic endometrium and hTERT expression in normal endometrium, and to analyze the correlation between hTERT expression and CA125 r-AFS stage and age in endometriosis. Results: the expression level of hTERT in eutopic endometrium in endometriosis was significantly higher than that in normal endometrium, and the expression level of hTERT in ectopic endometriosis was higher than that in eutopic endometrium. The expression of hTERT in endometrium was significantly higher than that in normal endometrium. The expression of hTERT in eutopic endometrium and normal endometrium is not significantly different from the periodic change of endometrium. The expression of p0.05 hTERT in eutopic endometrium is higher than that in normal endometrium in proliferative phase. The expression of p0.05 hTERT in eutopic endometrium in endometriosis during menstrual secretion was significantly enhanced compared with that in normal endometrium. There was no significant difference in the expression of p0.05T between eutopic endometrium and normal endometrium. There was no significant difference between the expression of hTERT and the change of menstrual cycle in normal endometrium and eutopic endometrium. The expression of hTERT in endometriosis was positively correlated with the level of CA125 in serum, but not with r-AFS staging (p0.05) and age (p0.05). Conclusion the abnormal expression of hTERT in endometriosis suggests that hTERT may play a role in the pathogenesis of endometriosis. The occurrence of endometriosis may be related to the abnormal proliferation of eutopic endometrial cells and ectopic cells. The immunohistochemical staining of hTERT combined with serum CA125 may be useful in evaluating the malignant change of endometriosis.
