【摘要】：目的分析子宮平滑肌脂肪瘤的臨床、病理及MRI特點(diǎn)。方法回顧性分析2010-2012年中國(guó)人民解放軍第105醫(yī)院2例經(jīng)手術(shù)病理證實(shí)的子宮平滑肌脂肪瘤病例資料,結(jié)合文獻(xiàn)中資料較為完整的27例進(jìn)行綜合分析。結(jié)果 1子宮平滑肌脂肪瘤主要發(fā)生于圍絕經(jīng)期或絕經(jīng)后婦女,臨床大多有不規(guī)則陰道流血、下腹脹痛等癥狀或者無明顯癥狀。21例子宮平滑肌脂肪瘤位于肌壁間,4例位于黏膜下,2例位于漿膜下,1例位于宮頸,1例位于左側(cè)闊韌帶。2顯微鏡下瘤體由成熟平滑肌細(xì)胞和脂肪細(xì)胞按不同比例混合構(gòu)成。3MRI信號(hào)特點(diǎn):子宮平滑肌脂肪瘤大部分以T1、T2高信號(hào),脂肪抑制序列為低信號(hào);增強(qiáng)后病灶內(nèi)非脂肪信號(hào)軟組織成分呈輕到中度強(qiáng)化;多無囊變或壞死。結(jié)論子宮平滑肌脂肪瘤好發(fā)生于圍絕經(jīng)期或絕經(jīng)后婦女,臨床表現(xiàn)與子宮平滑肌瘤類似,腫瘤內(nèi)大范圍的脂肪信號(hào)是其MRI特點(diǎn)。
[Abstract]:Objective to analyze the clinical, pathological and MRI features of uterine leiomyolipoma. Methods two cases of uterine leiomyolipoma confirmed by surgery and pathology in the 105th Hospital of Chinese people's Liberation Army from 2010 to 2012 were retrospectively analyzed. Results (1) uterine leiomyolipoma mainly occurred in perimenopausal or postmenopausal women. 21 cases of uterine leiomyolipoma located between muscle walls 4 cases located in submucous membrane 2 cases in subserous membrane 1 case in cervix and 1 case in left broad ligament .2 microscope the tumor was matured and smooth. The characteristics of MRI signal of myocytes and adipocytes mixed in different proportions were as follows: most of the uterine leiomyolipoma had high signal intensity in T _ 1 and T _ 2. The sequence of fat suppression was low signal; the soft tissue component of non-fat signal was mild to moderate enhancement after enhancement, and there was no cystic degeneration or necrosis. Conclusion uterine leiomyolipoma is more common in perimenopausal or postmenopausal women, and its clinical manifestation is similar to that of uterine leiomyoma. The large range of fat signal in the tumor is the characteristic of MRI.
