【摘要】：目的觀察甲羥孕酮片聯(lián)合甲硝唑和頭孢曲松治療子宮內(nèi)膜炎的臨床療效及其對(duì)相關(guān)細(xì)胞因子的影響。方法將96例子宮內(nèi)膜炎患者隨機(jī)分為試驗(yàn)組與對(duì)照組,各48例。對(duì)照組給予注射用頭孢曲松鈉0.5 g,注射用甲硝唑25mg,每天1次,分別靜脈滴注,連續(xù)治療14 d。試驗(yàn)組在對(duì)照組的基礎(chǔ)上給予醋酸甲羥孕酮片2 mg,每天1次,口服,連續(xù)服用21 d。2組均連續(xù)治療2個(gè)月經(jīng)周期。比較2組患者的臨床療效、臨床癥狀消退時(shí)間、炎癥因子變化及藥物不良反應(yīng)發(fā)生情況。結(jié)果治療后,試驗(yàn)組和對(duì)照組的總有效率分別為95.83%(46/48例),81.25%(39/48例),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療后,試驗(yàn)組陰道出血、下腹墜痛、脹痛臨床癥狀消退時(shí)間分別為(3.22±0.91),(5.43±2.16),(2.41±1.65)d,對(duì)照組分別為(5.18±1.35),(6.56±2.14),(5.26±1.87)d,差異均有統(tǒng)計(jì)學(xué)意義(均P0.05)。治療后,試驗(yàn)組白細(xì)胞介素-2(IL-2)、白細(xì)胞介素-4(IL-4)及白細(xì)胞介素-10(IL~(-1)0)分別為(4.28±1.06),(8.06±1.03),(20.25±4.19)μg·L~(-1),對(duì)照組分別為(3.76±0.97),(9.04±0.91),(18.38±3.64)μg·L~(-1),差異均有統(tǒng)計(jì)學(xué)意義(均P0.05)。試驗(yàn)組出現(xiàn)頭暈1例,困倦2例,藥物不良反應(yīng)發(fā)生率為6.25%(3/48例);對(duì)照組出現(xiàn)頭暈3例,困倦3例,嗜睡4例,藥物不良反應(yīng)發(fā)生率為20.83%(10/48例),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論頭孢曲松、甲硝唑聯(lián)合甲羥孕酮可迅速緩解子宮內(nèi)膜炎患者各項(xiàng)臨床癥狀,維持炎癥因子的平衡,提高臨床療效,且藥物不良反應(yīng)輕微。
[Abstract]:Objective to observe the clinical effect of medroxyprogesterone tablets combined with metronidazole and ceftriaxone in the treatment of endometritis and its effect on cytokines. Methods 96 cases of endometritis were randomly divided into experimental group and control group, 48 cases in each group. The control group was given ceftriaxone sodium 0.5 g for injection and metronidazole 25 mg for injection once a day for 14 days. The experimental group was given medroxyprogesterone acetate 2 mg once a day on the basis of control group. The clinical efficacy, the time of clinical symptom regression, the changes of inflammatory factors and the occurrence of adverse drug reactions were compared between the two groups. Results after treatment, the total effective rates of the test group and the control group were 95.83 / 46 / 48 / 81.25 / 39 / 48, respectively. The difference was statistically significant (P 0.05). After treatment, the clinical symptoms of vaginal bleeding, lower abdominal pain and flatulence in the experimental group were 3.22 鹵0.91 and 5.41 鹵1.65 days, respectively, and those in the control group were 5.18 鹵1.35, 6.56 鹵2.14 and 5.26 鹵1.87 days, respectively. The difference was statistically significant (P 0.05). After treatment, the levels of Interleukin-2, IL-4 and IL-10 were 4.28 鹵1.06 鹵1.036 鹵20.25 鹵4.19 渭 g / L, respectively, in the experimental group and 3.76 鹵0.97 鹵0.97 鹵0.91g / L, respectively, with statistical significance (P 0.05). There were 1 case of dizziness and 2 cases of sleepiness in the trial group, and the incidence of adverse drug reaction was 6.25% or 3 / 48 cases, while in the control group there were 3 cases of dizziness, 3 cases of sleepiness, 4 cases of drowsiness, and the incidence of adverse drug reaction was 20.833% 10 / 48 cases (P 0.05). Conclusion ceftriaxone, metronidazole and medroxyprogesterone can relieve the clinical symptoms of endometritis, maintain the balance of inflammatory factors, improve the clinical efficacy, and the adverse drug reaction is slight.
