【摘要】：目的:評(píng)價(jià)絕經(jīng)后女性子宮內(nèi)膜細(xì)胞學(xué)取材滿意度和應(yīng)用子宮內(nèi)膜細(xì)胞學(xué)檢查(ECT)診斷子宮內(nèi)膜癌及癌前病變的準(zhǔn)確性。方法:1045例絕經(jīng)后女性采用子宮內(nèi)膜細(xì)胞學(xué)取材器(SAP-1)刷取細(xì)胞學(xué)標(biāo)本,222例進(jìn)行宮腔鏡下活檢或分段診刮(DC)術(shù)或子宮全切除術(shù)獲取組織標(biāo)本。比較兩種方法采集標(biāo)本的滿意度情況,及絕經(jīng)年限和子宮內(nèi)膜厚度對(duì)兩種取材法標(biāo)本滿意度的影響,另對(duì)ECT的診斷效能進(jìn)行判斷。結(jié)果:11045例細(xì)胞學(xué)標(biāo)本中僅39例(3.7%)不充分,222例女性子宮內(nèi)膜組織學(xué)標(biāo)本中82例(36.9%)不充分,兩者差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2細(xì)胞學(xué)標(biāo)本充分組和不充分組病例的絕經(jīng)年限、子宮內(nèi)膜厚度的均值比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。組織學(xué)標(biāo)本充分組與不充分組病例的絕經(jīng)年限、子宮內(nèi)膜厚度的均值比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),組織學(xué)不充分組絕經(jīng)年限≥10年和子宮內(nèi)膜厚度≤4 mm的病例更多見(P0.05)。3ECT檢查診斷絕經(jīng)后女性子宮內(nèi)膜癌及不典型增生的準(zhǔn)確度是93.4%,敏感度、特異度、陽(yáng)性預(yù)測(cè)值、陰性預(yù)測(cè)值分別為72.4%、99.1%、95.5%、93.0%。結(jié)論:細(xì)胞學(xué)取材術(shù)比組織學(xué)取材能提供更充分的標(biāo)本,ECT可能成為篩查絕經(jīng)后女性子宮內(nèi)膜癌及癌前病變可靠的方法。
[Abstract]:Objective: to evaluate the degree of satisfaction of endometrial cytology in postmenopausal women and the accuracy of endometrial cytology in the diagnosis of endometrial carcinoma and precancerous lesions. Methods one hundred and forty-five postmenopausal women were brushed by endometrial cytology (SAP-1) to obtain tissue specimens from 222 cases of cytological specimens obtained by hysteroscopic biopsy or segmental curettage or total hysterectomy. The degree of satisfaction of the two methods was compared, and the influence of menopausal age and endometrial thickness on the satisfaction of the two methods was compared. The diagnostic efficacy of ECT was also evaluated. Results only 39 out of 11 045 cytological specimens were inadequate and 82 of 222 female endometrial specimens were not adequate. The difference between them was statistically significant (P 0.05.2). There was no significant difference in endometrial thickness between the two groups (P 0.05). The mean value of endometrial thickness and menopausal age of patients with adequate histological specimen and inadequate group were compared. The difference was statistically significant (P 0.05). The accuracy of P0.05 + 3 ECT in diagnosing postmenopausal women endometrial carcinoma and atypical hyperplasia was 93. 4% with sensitivity and specificity in the cases with menopausal age 鈮，
