【摘要】：目的:評價絕經后女性子宮內膜細胞學取材滿意度和應用子宮內膜細胞學檢查(ECT)診斷子宮內膜癌及癌前病變的準確性。方法:1045例絕經后女性采用子宮內膜細胞學取材器(SAP-1)刷取細胞學標本,222例進行宮腔鏡下活檢或分段診刮(DC)術或子宮全切除術獲取組織標本。比較兩種方法采集標本的滿意度情況,及絕經年限和子宮內膜厚度對兩種取材法標本滿意度的影響,另對ECT的診斷效能進行判斷。結果:11045例細胞學標本中僅39例(3.7%)不充分,222例女性子宮內膜組織學標本中82例(36.9%)不充分,兩者差異有統(tǒng)計學意義(P0.05)。2細胞學標本充分組和不充分組病例的絕經年限、子宮內膜厚度的均值比較,差異均無統(tǒng)計學意義(P0.05)。組織學標本充分組與不充分組病例的絕經年限、子宮內膜厚度的均值比較,差異有統(tǒng)計學意義(P0.05),組織學不充分組絕經年限≥10年和子宮內膜厚度≤4 mm的病例更多見(P0.05)。3ECT檢查診斷絕經后女性子宮內膜癌及不典型增生的準確度是93.4%,敏感度、特異度、陽性預測值、陰性預測值分別為72.4%、99.1%、95.5%、93.0%。結論:細胞學取材術比組織學取材能提供更充分的標本,ECT可能成為篩查絕經后女性子宮內膜癌及癌前病變可靠的方法。
[Abstract]:Objective: to evaluate the degree of satisfaction of endometrial cytology in postmenopausal women and the accuracy of endometrial cytology in the diagnosis of endometrial carcinoma and precancerous lesions. Methods one hundred and forty-five postmenopausal women were brushed by endometrial cytology (SAP-1) to obtain tissue specimens from 222 cases of cytological specimens obtained by hysteroscopic biopsy or segmental curettage or total hysterectomy. The degree of satisfaction of the two methods was compared, and the influence of menopausal age and endometrial thickness on the satisfaction of the two methods was compared. The diagnostic efficacy of ECT was also evaluated. Results only 39 out of 11 045 cytological specimens were inadequate and 82 of 222 female endometrial specimens were not adequate. The difference between them was statistically significant (P 0.05.2). There was no significant difference in endometrial thickness between the two groups (P 0.05). The mean value of endometrial thickness and menopausal age of patients with adequate histological specimen and inadequate group were compared. The difference was statistically significant (P 0.05). The accuracy of P0.05 + 3 ECT in diagnosing postmenopausal women endometrial carcinoma and atypical hyperplasia was 93. 4% with sensitivity and specificity in the cases with menopausal age 鈮，
