【摘要】：目的:探討Ⅰ型子宮內(nèi)膜癌患者盆腔淋巴結(jié)轉(zhuǎn)移相關(guān)危險(xiǎn)因素,為制定合理手術(shù)范圍提供依據(jù)。方法:對(duì)136例Ⅰ型子宮內(nèi)膜癌患者淋巴結(jié)轉(zhuǎn)移的危險(xiǎn)因素進(jìn)行分析。單因素采用χ~2檢驗(yàn)或Fisher確切概率法。多因素采用Logistic回歸模型。結(jié)果:136例Ⅰ型子宮內(nèi)膜癌患者盆腔淋巴結(jié)陽性率9.56%(13/136)。單因素分析表明Ⅰ型子宮內(nèi)膜癌的組織學(xué)分級(jí)、癌灶直徑大小、肌層浸潤深度、脈管浸潤與淋巴結(jié)轉(zhuǎn)移有關(guān)(P0.05);多因素Logistic回歸模型分析顯示組織低分化、肌層浸潤深度≥1/2、癌灶直徑≥2 cm、有脈管浸潤是Ⅰ型子宮內(nèi)膜癌盆腔淋巴結(jié)轉(zhuǎn)移的獨(dú)立危險(xiǎn)因素(P0.05)。結(jié)論:Ⅰ型子宮內(nèi)膜癌患者盆腔淋巴結(jié)轉(zhuǎn)移率低。組織低分化、肌層浸潤深度≥1/2、癌灶直徑≥2 cm、有脈管浸潤的Ⅰ型子宮內(nèi)膜癌患者更易發(fā)生盆腔淋巴結(jié)轉(zhuǎn)移。
[Abstract]:Objective: To explore the risk factors of pelvic lymph node metastasis in patients with type I endometrial carcinoma, and to provide the basis for making a reasonable operation range. Methods: the risk factors of lymph node metastasis in 136 patients with endometrial carcinoma were analyzed. The single factor was determined by the X ~2 test or the exact Fisher probability method. The multiple factors adopted the Logistic regression model. The results were as follows: The positive rate of pelvic lymph nodes in 136 patients with type I endometrial carcinoma was 9.56% (13/136). The single factor analysis showed that the histological grade of type I endometrial carcinoma, the diameter of the tumor, the depth of the myometrium infiltration, the infiltration of the vasculature with the lymph node metastasis (P0.05); the multifactor Logistic regression model showed that the tissue was low differentiation, the depth of the myometrium infiltration was more than 1/2, and the carcinoma was more than 1/2. The diameter of the focal diameter is more than 2 cm, and the infiltration of the pulse tube is an independent risk factor (P0.05) for the pelvic lymph node metastasis of type I endometrial carcinoma. Conclusion: the pelvic lymph node metastasis rate of type I endometrium cancer patients is low. The tissue is low differentiation, the depth of the myometrium infiltration is more than 1/2, the diameter of the cancer is more than 2 cm, and the patients with type I endometrium cancer are more prone to pelvic lymphadenocarcinoma.
