【摘要】：目的總結(jié)胎兒淚囊突出(DCC)的超聲圖像特征和產(chǎn)前超聲診斷的臨床價(jià)值。方法對(duì)在我院產(chǎn)前超聲檢查檢出的14例先天性DCC的超聲影像表現(xiàn)、合并異常及臨床隨訪結(jié)果進(jìn)行總結(jié)分析。結(jié)果胎兒先天性DCC超聲圖像表現(xiàn):(1)胎兒眼眶內(nèi)側(cè)稍下方可見(jiàn)邊界清晰的類圓形囊性病變,彩色多普勒超聲顯示囊性病變內(nèi)無(wú)血流信號(hào)。(2)部分胎兒(4/14)囊性病變內(nèi)可見(jiàn)點(diǎn)狀強(qiáng)回聲。(3)首次發(fā)現(xiàn)DCC時(shí)孕周為25~35周,平均27.6周。(4)以單側(cè)和女性胎兒多見(jiàn)(9/14)。(5)單發(fā)淚囊突出多見(jiàn)(12/14)。(6)產(chǎn)前超聲復(fù)查發(fā)現(xiàn)5例(5/14)淚囊突出消失。(7)2例(2/14)出生后出現(xiàn)淚囊炎,經(jīng)抗生素治療或淚囊探通術(shù)后緩解。其余12例出生后均無(wú)明顯眼部癥狀。結(jié)論胎兒淚囊突出產(chǎn)前超聲診斷準(zhǔn)確,宮內(nèi)緩解率高。胎兒DCC及合并異常應(yīng)進(jìn)行隨訪,對(duì)早期治療有指導(dǎo)意義。
[Abstract]:Objective to summarize the ultrasonographic features of fetal lacrimal sac protrusion (DCCs) and the clinical value of prenatal ultrasound diagnosis. Methods the ultrasonographic findings of 14 cases of congenital DCC diagnosed by prenatal ultrasound in our hospital were summarized and analyzed. Results the fetal congenital DCC imaging showed that the fetus had circular cystic lesions with clear boundary just below the medial side of the orbit. Color Doppler ultrasound showed no blood flow signal in cystic lesions. (14 / 14) in some fetal lesions, there were dotted hyperechoic echoes in cystic lesions.) the gestational week was 2535 weeks when DCC was first found. The average value of 27.6 weeks was 27.6 weeks. 4) unilateral and female fetuses were more common in 9 / 14. 5) single lacrimal sac herniation was 12 / 14%. 6) Prenatal ultrasound reexamination found 5 / 14) dacryocystitis disappeared in 2 cases (2 / 14). Dacryocystitis occurred after birth and was relieved after antibiotic treatment or dacryocystorhinostomy. The other 12 cases had no obvious ocular symptoms after birth. Conclusion the diagnosis of fetal lacrimal sac protrusion by prenatal ultrasound is accurate and the rate of intrauterine remission is high. Fetal DCC and associated abnormalities should be followed up, which has guiding significance for early treatment.
