【摘要】：目的報(bào)道1例以不明原因發(fā)熱為首發(fā)癥狀的Krukenberg瘤病例,提高對(duì)Krukenberg瘤的認(rèn)識(shí)。方法分析1例42歲Krukenberg瘤患者的病例資料。結(jié)果患者以不明原因發(fā)熱入院,入院時(shí)考慮肺炎,經(jīng)抗感染治療后效果不明顯。胸部CT平掃縱膈窗示胃壁增厚、腹腔及腹膜后淋巴結(jié)腫大,電子胃鏡檢查示胃癌,子宮及附件超聲示盆腔積液、腹膜后淋巴結(jié)腫大。擇期行姑息性胃大部切除術(shù)+腹腔探查術(shù),術(shù)中發(fā)現(xiàn)雙側(cè)卵巢轉(zhuǎn)移癌,予以姑息性胃大部切除及雙側(cè)卵巢切除。術(shù)后患者體溫恢復(fù)正常,術(shù)后4個(gè)月死于肝臟多發(fā)轉(zhuǎn)移及惡性腹腔積液。結(jié)論Krukenberg瘤可以不明原因發(fā)熱為首發(fā)癥狀,對(duì)女性胃腸道腫瘤患者診斷時(shí)應(yīng)考慮到Krukenberg瘤可能。
[Abstract]:Objective to report a case of Krukenberg tumor with unknown fever as its initial symptom and to improve the understanding of Krukenberg tumor. Methods A 42-year-old patient with Krukenberg's tumor was analyzed. Results the patients were admitted to hospital with unknown fever and pneumonia was taken into account. The effect was not obvious after anti-infection treatment. The mediastinal window of chest CT showed the thickening of gastric wall, the enlargement of abdominal and retroperitoneal lymph nodes, the appearance of gastric cancer by electronic gastroscopy, the pelvic effusion of uterus and appendages, and the enlargement of retroperitoneal lymph nodes. Palliative subtotal gastrectomy was performed. Bilateral metastatic ovarian carcinoma was found during the operation. Palliative subtotal gastrectomy and bilateral ovariectomy were performed. The body temperature returned to normal after operation and died of multiple liver metastasis and malignant peritoneal effusion 4 months after operation. Conclusion Fever of unknown origin may be the initial symptom of Krukenberg tumor. The possibility of Krukenberg tumor should be taken into account in the diagnosis of female gastrointestinal tumor.
