【摘要】：目的研究多囊卵巢綜合征(PCOS)患者臨床干預(yù)前后血清內(nèi)脂素水平變化及其在PCOS病理生理機(jī)制中可能發(fā)揮的作用。方法選取2012年8月-2013年9月該院生殖醫(yī)學(xué)科71例PCOS患者為PCOS組,45例健康志愿者或在生殖醫(yī)學(xué)中心行輔助治療的為對(duì)照組。比較血清內(nèi)脂素水平及相關(guān)因素;根據(jù)PCOS患者不同BMI及胰島素抵抗指數(shù)(HOMA-IR)分組,檢測(cè)不同組間血清內(nèi)脂素水平差異;比較38例PCOS患者治療前后血清內(nèi)脂素、BMI、睪酮(T)、LH/FSH值及HOMA-IR變化。結(jié)果 71例PCOS患者總體血清內(nèi)脂素水平低于對(duì)照組,但差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),內(nèi)脂素水平與HOMA-IR呈低度正相關(guān)(r=0.385,P0.01);對(duì)照組內(nèi)脂素水平未發(fā)現(xiàn)與其他因素有明顯相關(guān)性。PCOS各組內(nèi)脂素水平和對(duì)照組無(wú)差別,僅PCOS肥胖患者血清內(nèi)脂素水平顯著低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。PCOS治療后T、BMI、HOMA-IR及LH/FSH均顯著降低,治療前后差異均有統(tǒng)計(jì)學(xué)意義(P0.01),而內(nèi)脂素水平較治療前顯著升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。PCOS治療前后血清內(nèi)脂素水平變化與T、BMI、HOMA-IR及LH/FSH的變化均無(wú)明顯相關(guān)性。結(jié)論肥胖PCOS患者內(nèi)脂素水平低于對(duì)照組,PCOS患者經(jīng)治療干預(yù)后內(nèi)分泌及代謝指標(biāo)均得到改善,但內(nèi)脂素水平升高,提示內(nèi)脂素可能在改善PCOS患者臨床指標(biāo)上發(fā)揮作用。
[Abstract]:Objective to study the changes of serum lipid level in patients with polycystic ovary syndrome (PCOS) before and after clinical intervention and its possible role in the pathophysiological mechanism of PCOS. Methods from August 2012 to September 2013, 71 PCOS patients were selected as PCOS group (45 healthy volunteers) or as control group. The serum lipid levels and related factors were compared according to the different BMI and insulin resistance index (HOMA-IRR) of patients with PCOS, and the changes of serum lipopolysaccharide (BMI), testosterone (T) and HOMA-IR in 38 patients with PCOS before and after treatment were compared. Results the serum lipid levels in 71 patients with PCOS were lower than those in the control group. However, there was no significant difference (P 0.05), and the level of endolipidin was positively correlated with HOMA-IR (0.385% P 0.01), but there was no significant correlation between the level of lipid in the control group and other factors. There was no significant difference between the levels of lipoprotein in each group and that in the control group, but there was no significant difference between the two groups in the level of endolipidin and that in the control group. The serum levels of lipoprotein in obese patients with PCOS were significantly lower than those in the control group. The difference was statistically significant (P 0.05). The levels of HOMA-IR and LH/FSH in patients with PCOS were significantly lower than those in the control group. The difference before and after treatment was statistically significant (P 0.01), while the level of endolipidin was significantly higher than that before treatment. There was no significant correlation between the changes of serum lipid level and the changes of TBMIHOMA-IR and LH/FSH before and after treatment. Conclusion Endocrine and metabolic indexes in obese PCOS patients were lower than those in the control group, but the levels of endolipidins increased, suggesting that endolipidins may play an important role in improving the clinical indexes of PCOS patients.
