微刺激全部胚胎冷凍方案在常規(guī)體外受精-胚胎移植失敗的卵巢低反應(yīng)患者中的應(yīng)用

【摘要】:目的探討微刺激全部胚胎冷凍方案在常規(guī)體外受精/卵胞質(zhì)內(nèi)單精子顯微注射-胚胎移植(IVF/ICSI-ET)方案失敗的卵巢低反應(yīng)(POR)患者中的應(yīng)用價(jià)值。方法對(duì)196例IVF/ICSI-ET應(yīng)用常規(guī)促排卵方案因POR放棄周期或移植失敗后改用微刺激方案的402個(gè)周期進(jìn)行回顧性分析,比較前、后2次不同促排卵方案的臨床和實(shí)驗(yàn)室指標(biāo)。結(jié)果 402個(gè)微刺激周期中32個(gè)周期取消,周期取消率為7.96%,370個(gè)周期獲得卵母細(xì)胞,平均獲卵數(shù)(2.2±1.5)個(gè),共233個(gè)周期有胚胎冷凍,胚胎冷凍率為57.96%,共194個(gè)周期進(jìn)行了ET,臨床妊娠74例,早期自然流產(chǎn)6例,起始周期累積妊娠率為37.75%(74/196),凍融胚胎移植(FET)周期臨床妊娠率為38.14%(74/194),其中≤37歲組FET周期的臨床妊娠率(50.89%)明顯高于37歲組(20.73%)(P0.05)。微刺激方案的臨床和實(shí)驗(yàn)室指標(biāo)均顯著優(yōu)于常規(guī)長方案。結(jié)論微刺激全部胚胎冷凍方案對(duì)常規(guī)方案IVF/ICSI-ET治療失敗的POR患者具有療程短、刺激劑量小,獲卵率高、可利用胚胎率高、周期取消率低等特點(diǎn),對(duì)于該類患者再次助孕可考慮微刺激方案。
[Abstract]:Objective to investigate the value of microstimulation of all embryo freezing protocols in patients with ovarian hyporesponse (POR) after IVF / ICSI-ETT failed in vitro fertilization / intracytoplasmic sperm microinjection (IVF / ICSI-ETT). Methods the clinical and laboratory indexes of 196 patients with IVF/ICSI-ET who were treated with conventional ovulation stimulation regimen because of POR abandoning cycle or failed transplantation were analyzed retrospectively. The clinical and laboratory indexes of different ovulation promotion protocols were compared before and after the ovulatory therapy. Results 32 cycles of 402 microstimulation cycles were cancelled, the cycle cancellation rate was 7.96. The oocytes were obtained in 370 cycles. The average number of oocytes was 2.2 鹵1.5. There were 233 cycles with embryo freezing. The embryo freezing rate was 57.96. ETS were performed in 194 cycles. 74 clinical pregnancies were performed. In 6 cases of early spontaneous abortion, the cumulative pregnancy rate of initial cycle was 37.75 / 74 / 196, and the clinical pregnancy rate of frozen-thawed embryo transfer transfer (fet) cycle was 38.14 / 194%.
