【摘要】：目的探討合并心臟病的孕產(chǎn)婦發(fā)生心力衰竭的危險(xiǎn)因素。方法回顧分析于河南省新鄉(xiāng)醫(yī)學(xué)院第一附屬醫(yī)院接受治療的344例合并心臟病的孕產(chǎn)婦的臨床資料,發(fā)生心力衰竭的61例患者作為觀察組,同期未發(fā)生心力衰竭的258例患者作為對(duì)照組。給予發(fā)生心力衰竭的孕產(chǎn)婦急救治療,并分別采用單因素、多因素Logistic回歸法分析合并心臟病孕產(chǎn)婦并發(fā)心力衰竭的危險(xiǎn)因素。結(jié)果基礎(chǔ)心率100次/min、先天性心臟病、圍生期心肌病、風(fēng)濕性心臟病、PAP50 mm Hg、年齡≥35歲、孕前發(fā)生心臟事件、早孕期心功能分級(jí)≥Ⅱ級(jí)是心臟病孕產(chǎn)婦發(fā)生心力衰竭的獨(dú)立危險(xiǎn)因素(P0.05),而孕前咨詢和產(chǎn)前檢查≥6次是其保護(hù)性因素(P0.05)。結(jié)論合并心臟病孕產(chǎn)婦并發(fā)心力衰竭受多種因素影響,在臨床中應(yīng)對(duì)危險(xiǎn)因素干預(yù)或規(guī)避,并幫助患者積極實(shí)現(xiàn)保護(hù)性因素,從而降低心力衰竭的發(fā)生率。
[Abstract]:Objective to investigate the risk factors of heart failure in pregnant women with heart disease. Methods Clinical data of 344 pregnant women with heart disease were retrospectively analyzed in the first affiliated Hospital of Xinxiang Medical College of Henan Province. 61 patients with heart failure were treated as observation group. In the same period, 258 patients without heart failure were used as control group.
