【摘要】：目的探討臍帶繞頸深度與大腦中動脈(MCA)及臍動脈(UA)血流參數(shù)的關(guān)系。方法選取孕晚期臍帶繞頸胎兒139胎及正常胎兒28胎。采用多普勒超聲檢測胎兒MCA及UA的收縮期最大速度/舒張最小速度(S/D)、阻力指數(shù)(RI)、搏動指數(shù)(PI),并分析臍帶繞頸胎兒壓跡深度與MCA、UA血流參數(shù)的相關(guān)性。結(jié)果臍帶繞頸胎兒壓跡深度與MCA的S/D、RI、PI呈正相關(guān)(繞頸1周r=0.32、0.32、0.35,繞頸2周r=0.71、0.69、0.53;P均0.05),與UA的S/D、RI、PI無相關(guān)性。臍帶繞頸胎兒與正常胎兒UA血流參數(shù)差異無統(tǒng)計學(xué)意義(P均0.05),臍帶繞頸1周壓跡深度≥0.80cm及臍帶繞頸2周及以上胎兒MCA的S/D、RI、PI均高于正常胎兒(P均0.05)。與正常胎兒比較,臍帶繞頸2周胎兒的剖宮產(chǎn)率明顯增高,臍帶繞頸1周且壓跡深度≥0.80cm的胎兒胎心監(jiān)護(hù)異常率升高,第二產(chǎn)程時間延長,差異均有統(tǒng)計學(xué)意義(P均0.05)。結(jié)論孕晚期臍帶繞頸胎兒的MCA血流參數(shù)較UA更敏感,繞頸1周壓跡深度≥0.80cm可能引起胎兒宮內(nèi)缺氧。
[Abstract]:Objective to investigate the relationship between the depth of umbilical cord around neck and the blood flow parameters of MCA and UAA. Methods Thirty nine fetuses with umbilical cord around neck and 28 normal fetuses were selected. The maximal systolic / diastolic velocities of fetal MCA and UA were measured by Doppler ultrasound, and the resistance index (RI) and pulsatile index (Pi) were measured. The correlation between fetal indentation depth around the neck of umbilical cord and the blood flow parameters of UA was analyzed. Results there was a positive correlation between the fetal indentation depth around the neck of umbilical cord and the S / D Pi of MCA (r = 0.32 鹵0.32 ~ 0.35, r = 0.71 ~ 0. 69 ~ 0. 53 P = 0. 05), respectively. There was no correlation between the depth of umbilical cord pressure and the Pi of S / D ~ (+) I ~ (1) of UA.
