【摘要】：目的：本研究通過觀察經(jīng)腹腔鏡下卵巢子宮內(nèi)膜異位囊腫剝除手術(shù)后的患者，分別采用GnRH-a、中藥治療的臨床療效，復(fù)發(fā)率及妊娠率，探討中醫(yī)藥治療本病的臨床價(jià)值。 方法：2012年2月至2013年2月在河南中醫(yī)學(xué)院第一附屬醫(yī)院婦產(chǎn)科行腹腔鏡下卵巢子宮內(nèi)膜異位囊腫剝除手術(shù)，術(shù)后病理確定符合卵巢子宮內(nèi)膜異位癥的有生育要求的患者60例。隨機(jī)分為治療組（中藥治療）及對照組（GnRH-a治療）。所有患者隨訪12個(gè)月。觀察兩組患者治療后臨床療效、術(shù)后1年內(nèi)妊娠情況及復(fù)發(fā)情況。 結(jié)果：治療后兩組總有效率，治療組86.7%，對照組73.3%。兩組比較，P0.05，差異有統(tǒng)計(jì)學(xué)意義，術(shù)后中藥治療臨床療效優(yōu)于GnRH-a治療組；兩組均可改善患者的痛經(jīng)、慢性盆腔痛及性交痛癥狀，各組治療前后比較，P0.05，差異有統(tǒng)計(jì)學(xué)意義，，組間比較，P0.05，差異無統(tǒng)計(jì)學(xué)意義；術(shù)后1年內(nèi)治療組妊娠率83.3%，對照組56.7%，兩組比較，P0.05，差異顯著，治療組優(yōu)于對照組。并且術(shù)后妊娠率隨臨床期別加重而降低；復(fù)發(fā)率：術(shù)后1年內(nèi)復(fù)發(fā)率，治療組與對照組比較，P0.05，差異有統(tǒng)計(jì)學(xué)意義，治療組優(yōu)于對照組。 結(jié)論： 腹腔鏡手術(shù)聯(lián)合中藥治療有生育要求的卵巢型子宮內(nèi)膜異位癥患者，臨床療效優(yōu)于對照組，可有效改善痛經(jīng)、慢性盆腔痛及性交痛等癥狀，與對照組無明顯差異。但在提高術(shù)后1年內(nèi)的妊娠率，延長復(fù)發(fā)時(shí)間，降低復(fù)發(fā)率方面明顯優(yōu)于對照組，具有明顯的遠(yuǎn)期療效。
[Abstract]:Objective: to observe the clinical effect, recurrence rate and pregnancy rate of GnRH-aand the clinical value of traditional Chinese medicine (TCM) in the treatment of ovarian endometriosis cysts after laparoscopic excision.Methods: from February 2012 to February 2013, 60 patients with ovarian endometriosis were treated by laparoscopic excision of endometriosis in gynecology and obstetrics department of the first affiliated Hospital of Henan College of traditional Chinese Medicine.They were randomly divided into treatment group (Chinese medicine treatment) and control group (GnRH-a treatment group).All patients were followed up for 12 months.To observe the clinical effect, pregnancy and recurrence of the two groups after treatment.Results: the total effective rate was 86.7 in the treatment group and 73.3 in the control group.The difference between the two groups was statistically significant, the clinical efficacy of traditional Chinese medicine was better than that of GnRH-a group, the symptoms of dysmenorrhea, chronic pelvic pain and coitus pain were improved in both groups, and the difference was statistically significant before and after treatment.The pregnancy rate of the treatment group was 83.3%, and that of the control group was 56.7%. The difference between the two groups was significant (P 0.05), and the treatment group was superior to the control group.The recurrence rate of the treatment group was significantly higher than that of the control group, and the recurrence rate of the treatment group was better than that of the control group.Conclusion:Laparoscopic surgery combined with traditional Chinese medicine in the treatment of ovarian endometriosis with fertility requirements, the clinical efficacy is better than the control group, can effectively improve dysmenorrhea, chronic pelvic pain and intercourse pain and other symptoms, there is no significant difference between the control group and the control group.But it is superior to the control group in increasing pregnancy rate, prolonging recurrence time and reducing recurrence rate in one year after operation, and has obvious long-term curative effect.
