【摘要】：目的：通過Meta分析對血清HE4、CA125及兩者聯(lián)合診斷卵巢癌的價值進行探討。 方法：通過PubMed、Elsevier Science、Springer、中國知網(wǎng)、萬方及維普等數(shù)據(jù)庫檢索2006年1月至2013年12月的相關(guān)文獻，按照納入標準和排除標準對文獻進行嚴格篩選。提取納入文獻的資料特征及四格表，對納入研究使用QUADAS評價工具詳情表。進行質(zhì)量評價。使用Meta-Disc軟件1.4版進行統(tǒng)計學(xué)分析，分析及處理異質(zhì)性后選擇適當效應(yīng)模型合并效應(yīng)量，得出合并敏感性、合并特異性及合并似然比的森林圖，制作SROC曲線。 結(jié)果：初檢得到中、英文文獻共1046篇，篩選后20篇文獻可納入研究。經(jīng)Meta分析后得出合并效應(yīng)量：HE4的敏感度為0.70，特異度為0.92，陽性似然比為8.65，陰性似然比為0.31，SROC曲線下面積0.9116；CA125的敏感度為0.79，特異度為0.80，，陽性似然比為3.97，陰性似然比為0.28，SROC曲線下面積0.8678；兩者聯(lián)合使用的敏感度為0.88,特異度為0.75,陽性似然比為3.62,陰性似然比為0.16,SROC曲線下面積（AUC)為0.9064。 結(jié)論：HE4與CA125聯(lián)合使用對卵巢癌的診斷價值高于分別單獨使用，當患者血清HE4和CA125均為陰性時，排除卵巢癌的能力較強。
[Abstract]:Objective: to evaluate the value of serum HE4, CA125 and their combination in diagnosis of ovarian cancer by Meta analysis.Methods: the relevant literatures from January 2006 to December 2013 were searched through the database of PubMedus Elsevier Science Spring, China Zhiwang, Wanfang and Weip, and the literature was screened strictly according to the inclusion criteria and exclusion criteria.The data features and four grid tables of the literature were extracted, and the QUADAS evaluation tool was used for the inclusion study.Carry out quality evaluation.Statistical analysis was carried out by using Meta-Disc software version 1.4. After analyzing and dealing with heterogeneity, appropriate effect model was selected to combine effect quantity, and the forest map of combination sensitivity, combination specificity and merge likelihood ratio were obtained, and SROC curves were made.Results: a total of 1046 Chinese and English literatures were obtained, 20 of which could be included in the study.Meta analysis showed that the sensitivity, specificity, positive likelihood ratio, negative likelihood ratio under the SROC curve were 0.70, 0.92, 8.65, 0.79, 0.80, 3.97 and 0.28, respectively.The sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and area under SROC curve were 0.88, 0.75, 3.62 and 0.9064, respectively.Conclusion the diagnostic value of the combination of CA125 and He4 for ovarian cancer is higher than that used separately. When the serum HE4 and CA125 are both negative, the ability to exclude ovarian cancer is stronger.
