【摘要】：目的探討胎兒心臟超聲檢查在產(chǎn)前篩查中的臨床價值。方法選取1 872例孕22~28周的孕婦,采用GE VOLUSON E8彩色多普勒超聲診斷儀由胸部至腹部各個切面逐一掃查的方法,對胎兒心內(nèi)結(jié)構(gòu)及血流情況進(jìn)行系統(tǒng)篩查。將診斷為胎兒心臟發(fā)育異常的病例結(jié)果與孕婦引產(chǎn)后的尸檢結(jié)果和新生兒心臟超聲檢查結(jié)果進(jìn)行比較。結(jié)果 1 872例胎兒中共33例出現(xiàn)了心臟結(jié)構(gòu)異常的情況,其中經(jīng)產(chǎn)前胎兒心臟彩超篩查確診30例,漏診3例,胎兒心臟超聲檢查胎兒心臟畸形診斷準(zhǔn)確率為90.91%。漏診的為室間隔缺損(肌部)2例,肺靜脈異位引流1例。結(jié)論胎兒心臟超聲檢查作為一種經(jīng)濟(jì)、實(shí)用且無創(chuàng)的檢查手段,對于胎兒先天性心臟病的檢出率較高,有助于降低此類缺陷患兒的出生率,更好地落實(shí)優(yōu)生優(yōu)育政策。
[Abstract]:Objective to evaluate the clinical value of fetal echocardiography in prenatal screening.Methods A total of 1 872 pregnant women at 22 and 28 weeks of gestation were examined by GE VOLUSON E8 color Doppler echocardiography from chest to abdomen. The fetal heart structure and blood flow were screened systematically.The results of fetal cardiac dysplasia were compared with the results of postmortem examination after induced labor and the results of neonatal echocardiography.Results there were 33 cases of abnormal cardiac structure in 1 872 fetuses. Among them, 30 cases were diagnosed by prenatal fetal echocardiography, 3 cases missed diagnosis. The diagnostic accuracy of fetal heart malformation by fetal echocardiography was 90.91%.The missing diagnosis included ventricular septal defect (2 cases) and anomalous pulmonary venous drainage (1 case).Conclusion as an economical, practical and non-invasive method, fetal echocardiography has a high detection rate of congenital heart disease, which is helpful to reduce the birth rate of children with such defects and to carry out the policy of eugenics and breeding better.
