【摘要】：目的:探討夫精宮腔內(nèi)人工授精(IUI)時機(jī)及次數(shù)與妊娠率的關(guān)系。方法:回顧性分析851對在廣州醫(yī)科大學(xué)附屬第三醫(yī)院行夫精I(xiàn)UI治療的不孕夫婦(共1 751個周期),根據(jù)手術(shù)時機(jī)及次數(shù),分為排卵前1次IUI、排卵后1次IUI、排卵前2次IUI、排卵前后各1次IUI 4組,比較4組妊娠率的差異。結(jié)果:排卵前、后行單次IUI組的妊娠率分別為11.21%和12.23%;排卵前行單次IUI后有排卵組妊娠率顯著高于無排卵組(P0.05)。排卵前后各行1次IUI、排卵前行2次IUI妊娠率分別為16.21%和15.65%;排卵前行2次IUI后有排卵組臨床妊娠率(17.14%)高于無排卵組(0.00%),差異有統(tǒng)計學(xué)意義(確切概率法P=0.040)。有排卵周期中,排卵前1次、排卵后1次、排卵前2次、排卵前后各1次IUI的臨床妊娠率分別為10.99%、12.23%、17.14%、16.21%,行2次IUI組的妊娠率雖然高于行1次IUI組,但差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:只要確定卵泡破裂,排卵前后行IUI、行1次或2次IUI妊娠率無差異;但I(xiàn)UI后卵泡無破裂,則妊娠率為0~2.85%,因此針對2個周期IUI出現(xiàn)未破裂卵泡黃素化綜合征者建議轉(zhuǎn)行體外受精。
[Abstract]:Objective: to investigate the relationship between intrauterine intrauterine insemination (IUI) and pregnancy rate.Methods: a total of 851 infertile couples (1 751 cycles) treated with IUI in the third affiliated Hospital of Guangzhou Medical University were analyzed retrospectively.They were divided into four groups: one time before ovulation, one time after ovulation, two times before ovulation, and one time before and after ovulation.Results: the pregnancy rates of single IUI group were 11.21% and 12.23% respectively before ovulation, and the pregnancy rate of single IUI group before ovulation was significantly higher than that of non-ovulation group (P 0.05).The pregnancy rates of IUI before and after ovulation were 16.21% and 15.65, respectively, and the clinical pregnancy rate was 17.14% in the group with IUI before ovulation 2 times before ovulation, which was significantly higher than that in the group without ovulation (P < 0.040).In the ovulatory cycle, the clinical pregnancy rate of IUI before and after ovulation was 10.99 and 12.23and 17.14, respectively. The pregnancy rate in the second IUI group was higher than that in the first IUI group, but the difference was not statistically significant (P 0.05).Conclusion: as long as follicular rupture is determined, there is no difference in pregnancy rate between once or twice IUI before and after ovulation, but there is no rupture of follicle after IUI.The pregnancy rate is 0 and 2.85, so it is recommended that IVF be converted to IVF for those with unruptured follicular luteinization syndrome in 2 cycles of IUI.
