【摘要】：目的:探討妊娠期附件扭轉(zhuǎn)的臨床特點(diǎn)、診斷及手術(shù)治療。方法:回顧分析14例妊娠期附件扭轉(zhuǎn)患者的臨床資料。結(jié)果:14例患者中,行腹腔鏡手術(shù)者6例,開腹手術(shù)者8例;9例行患側(cè)附件切除術(shù),5例行患側(cè)卵巢囊腫剝除術(shù)。術(shù)后病理報(bào)告均為良性疾病,其中卵巢成熟性畸胎瘤4例,卵巢黃體囊腫3例,卵巢單純性囊腫3例,囊腺瘤2例,輸卵管系膜囊腫1例,附件廣泛出血淤血、未見明顯腫塊1例。6例患者足月分娩,新生兒健康。結(jié)論:妊娠期附件扭轉(zhuǎn)多為良性囊腫。早期診斷和治療有利于母兒預(yù)后,腹腔鏡是診斷和治療附件扭轉(zhuǎn)的可取方式。對生育年齡,有生育要求的婦女應(yīng)考慮保留附件的手術(shù)。
[Abstract]:Objective: to investigate the clinical features, diagnosis and surgical treatment of accessory torsion in pregnancy.Methods: the clinical data of 14 cases of pregnancy accessory torsion were analyzed retrospectively.Results among 14 patients, 6 cases underwent laparoscopic surgery, and 8 cases underwent laparoscopic surgery. 9 cases underwent appendage excision and 5 cases underwent ovarian cyst excision on the affected side.There were 4 cases of ovarian mature teratoma, 3 cases of ovarian luteal cyst, 3 cases of ovarian simple cyst, 2 cases of cystadenoma, 1 case of Mesangial cyst of fallopian tube, and extensive bleeding and congestion of adnexa.There was no obvious mass in 1 case (6 cases) with term delivery and newborn health.Conclusion: the accessory torsion in pregnancy is mostly benign cyst.Early diagnosis and treatment are beneficial to maternal and fetal prognosis. Laparoscopy is a desirable method for diagnosis and treatment of accessory torsion.For reproductive age, women with fertility requirements should consider surgery that preserves attachments.
