孕婦HBeAg陽(yáng)性對(duì)新生兒HBV血清學(xué)標(biāo)志物動(dòng)態(tài)變化的影響分析

【摘要】:目的分析HBeAg陽(yáng)性孕婦對(duì)于新生兒感染乙型肝炎病毒(HBV)情況的影響。方法選取2013年1月-2015年6月醫(yī)院接診的HBeAg陽(yáng)性孕婦共120名為感染組對(duì)象,根據(jù)HBV-DNA載毒量分為低毒亞組69例以及高毒亞組51例,選取醫(yī)院產(chǎn)檢分娩的HBeAg陰性孕婦120例為對(duì)照組,應(yīng)用PCR法檢測(cè)孕婦和新生兒的HBV-DNA表達(dá)量,應(yīng)用免疫組化法檢測(cè)乙型肝炎血清標(biāo)志物表達(dá)情況,對(duì)比組間新生兒HBV相關(guān)血清標(biāo)志物表達(dá)的差異,分析HBeAg陽(yáng)性以及孕婦HBV感染對(duì)新生兒的影響。結(jié)果感染組新生兒HBsAg陽(yáng)性率為29.17%、HBeAg陽(yáng)性率為39.17%、抗-HBc陽(yáng)性率為32.50%、大三陽(yáng)率為9.17%、小雙陽(yáng)率為33.33%、雙陽(yáng)率為8.33%、HBV-DNA陽(yáng)性率為51.67%,顯著高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);高毒亞組新生兒HBsAg陽(yáng)性率為49.02%、HBeAg陽(yáng)性率為50.98%、抗-HBc陽(yáng)性率為43.14%、大三陽(yáng)率為13.73%、小雙陽(yáng)率為54.90%、雙陽(yáng)率為15.69%、HBV-DNA陽(yáng)性率為66.67%,高于低毒亞組(P0.05);雙變量相關(guān)分析法顯示孕婦HBV-DNA表達(dá)量和新生兒HBV-DNA、HBsAg陽(yáng)性、HBeAg陽(yáng)性有顯著相關(guān)性(P0.01)。結(jié)論妊娠期間孕婦HBeAg陽(yáng)性能夠增加新生兒感染HBV的風(fēng)險(xiǎn),且孕婦HBV-DNA載毒量越高,新生兒感染HBV危險(xiǎn)越高,積極阻斷HBV母嬰傳播對(duì)于新生兒健康極為重要。
[Abstract]:Objective to analyze the effect of HBeAg positive pregnant women on neonatal hepatitis B virus infection.Methods from January 2013 to June 2015, 120 pregnant women with positive HBeAg were selected as infected group. According to the amount of HBV-DNA, 69 cases were divided into low toxic subgroup and 51 cases high toxic subgroup. 120 HBeAg negative pregnant women were selected as control group.PCR method was used to detect the expression of HBV-DNA in pregnant women and newborns, and immunohistochemical method was used to detect the expression of serum markers of hepatitis B. the difference of the expression of serum markers related to HBV in newborns was compared between the two groups.To analyze the influence of HBeAg positive and HBV infection on the newborn.Results the positive rate of HBsAg, anti-HBc and anti-HBc were 39.17, 32.50, 9.17, 33.33 and 8.33, respectively. The positive rate of HBV-DNA was significantly higher than that of the control group, and the positive rate of HBeAg was 39.17, the positive rate of anti-HBc was 32.50, the positive rate of anti-HBc was 9.17, the positive rate of HBeAg was 33.33 and the positive rate of HBV-DNA was 51.67, which was significantly higher than that in the control group.宸紓鏈夌粺璁″鎰忎箟(P0.05);楂樻瘨浜氱粍鏂扮敓鍎縃BsAg闃蟲(chóng),
