【摘要】：目的:分析盆底重建術(shù)后網(wǎng)片侵蝕暴露相關(guān)因素及遠(yuǎn)期臨床療效。方法:選取2007年1月至2014年4月在鄭州大學(xué)第三附屬醫(yī)院采用聚丙烯網(wǎng)片行盆底重建術(shù)的142例盆底器官脫垂患者,分析其術(shù)后復(fù)發(fā)及網(wǎng)片侵蝕暴露等并發(fā)癥情況。同時(shí)選取2006年2月至2014年2月因子宮脫垂或陰道前后壁膨出行傳統(tǒng)手術(shù)治療的68例盆底器官脫垂患者為對照。結(jié)果:盆底重建術(shù)后復(fù)發(fā)率為2.1%(3/142),傳統(tǒng)手術(shù)復(fù)發(fā)率為2.9%(2/68);盆底重建術(shù)的網(wǎng)片侵蝕暴露發(fā)生率為15.49%(22/142)。有糖尿病病史與盆底重建術(shù)后網(wǎng)片侵蝕暴露有關(guān)(P0.05),保留子宮則是其保護(hù)因素(P0.05)。使用網(wǎng)片的盆底重建術(shù)患者的術(shù)后并發(fā)癥發(fā)生率明顯高于對照組(P0.05),術(shù)后長期并發(fā)癥發(fā)生率則少于短期,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:利用聚丙烯網(wǎng)片進(jìn)行盆底重建術(shù)療效確切,復(fù)發(fā)率低;控制糖尿病及保留子宮的盆底重建手術(shù)可能降低術(shù)后網(wǎng)片侵蝕暴露的發(fā)生,術(shù)后長期臨床效果可能較短期有所改善。使用網(wǎng)片的盆底重建手術(shù)術(shù)后并發(fā)癥較傳統(tǒng)手術(shù)高。
[Abstract]:Objective: to analyze the related factors and long-term clinical effect of net erosion after pelvic floor reconstruction.Methods: from January 2007 to April 2014, 142 patients with pelvic floor prolapse underwent pelvic floor reconstruction with polypropylene mesh in the third affiliated Hospital of Zhengzhou University. The complications such as postoperative recurrence and net erosion exposure were analyzed.From February 2006 to February 2014, 68 patients with pelvic floor organ prolapse due to uterine prolapse or vaginal anterior and posterior wall prolapse were selected as control group.Results: the recurrence rate of pelvic floor reconstruction was 2. 1 / 142a, the recurrence rate of traditional operation was 2. 9 / 68, and the rate of net erosion in pelvic floor reconstruction was 15. 49- 22 / 142g.The history of diabetes was related to the net erosion exposure after pelvic floor reconstruction, and the preservation of uterus was the protective factor.The incidence of postoperative complications in patients with pelvic floor reconstruction using mesh was significantly higher than that in the control group (P 0.05), and the incidence of long-term complications was less than that in the short term. The difference was statistically significant (P 0.05).Conclusion: the pelvic floor reconstruction with polypropylene mesh is effective and the recurrence rate is low.The long-term clinical effect after operation may be better than that of short-term.The postoperative complications of pelvic floor reconstruction using mesh were higher than that of traditional surgery.
