【摘要】：目的觀察黃體期促排卵方案與Gn RH-a超短方案在行體外受精-胚胎移植(IVF-ET)助孕高齡患者中的應(yīng)用,探討黃體期促排卵方案的有效性及可行性。方法對114例高齡患者161個IVF-ET周期過程及結(jié)局進(jìn)行回顧性分析,其中,Gn RH-a超短方案83例(A組)、黃體期促排卵方案31例(B組),比較兩種方案的促排卵效果及妊娠結(jié)局。結(jié)果與A組相比,B組人絕經(jīng)期促性腺激素用量較多、重組促卵泡激素用量較少、促性腺激素(Gn)用量較多、Gn費(fèi)用較少及受精率較高,優(yōu)質(zhì)胚胎率較低,組間比較,差異有統(tǒng)計學(xué)意義(P0.05)。兩組患者在年齡、不孕年限、體重指數(shù)、基礎(chǔ)卵泡刺激素、基礎(chǔ)黃體生成素、基礎(chǔ)雌二醇、Gn天數(shù)、扳機(jī)日雌二醇水平、平均獲卵數(shù)、2PN、可利用胚胎數(shù)、可移植胚胎率、臨床妊娠率及早期流產(chǎn)率等方面比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論對于高齡患者,黃體期促排卵方案與Gn RH-a超短方案相比,在Gn費(fèi)用較少的情況下有較高的受精率,且兩組在總獲卵數(shù)、可利用胚胎率及妊娠結(jié)局等方面比較,差異無統(tǒng)計學(xué)意義(P0.05),提示黃體期促排卵方案可作為高齡IVF患者理想的治療方案。
[Abstract]:Objective To observe the luteal phase of ovulation induction with Gn RH-a ultra short scheme of in vitro fertilization and embryo transfer (IVF-ET) application of progesterone in elder patients, to explore the effectiveness and feasibility of the luteal phase for ovulation induction. Methods 114 cases of elderly patients with 161 cycles of IVF-ET process and outcome were retrospectively analyzed. Among them, Gn RH-a laser for 83 cases (group A), luteal phase ovulation for 31 cases (group B), ending a comparison of two schemes of ovulation induction and pregnancy. Results compared with A group, B group, human menopausal gonadotropin dosage, less dosage of recombinant follicle stimulating hormone, gonadotropin-releasing hormone (Gn) dosage. Gn cost less and higher fertilization rate, good embryo rate is low, comparison between groups, the difference was statistically significant (P0.05). The two groups in age, infertility duration, body mass index, follicle stimulating hormone, luteinizing hormone based, basic estradiol, Gn days, on the trigger Estradiol levels, the average number of oocytes, 2PN, number of available embryos, embryo transplant rate, compared the clinical pregnancy rate and early abortion rate, the difference was not statistically significant (P0.05). Conclusion for elderly patients, luteal phase ovulation scheme compared with the Gn RH-a scheme in Gn short, under the condition of less cost have a higher fertilization rate, and the two groups in the total number of oocytes, comparable by embryo rate and pregnancy outcome, there was no statistically significant difference (P0.05), suggesting that the luteal phase of ovulation induction regimen can be used as the treatment of elderly patients with IVF ideal.
