圍絕經(jīng)期及絕經(jīng)后女性應(yīng)用激素替代治療后對(duì)胰島素抵抗水平的影響

【摘要】:目的:研究圍絕經(jīng)期及絕經(jīng)后女性絕經(jīng)狀態(tài)與胰島素抵抗之間的關(guān)系,以及在應(yīng)用激素替代治療(HRT)后胰島素的改善情況。 方法:所有病例均來(lái)自于浙江大學(xué)附屬婦產(chǎn)科醫(yī)院更年期門診就診患者,根據(jù)其月經(jīng)情況將其分為圍絕經(jīng)早期、圍絕經(jīng)晚期及絕經(jīng)后組,分別測(cè)定其初診時(shí)內(nèi)分泌水平、空腹血糖及胰島素水平,并計(jì)算HOMA指數(shù);在其HRT6個(gè)月后再次復(fù)測(cè)上述指標(biāo)進(jìn)行對(duì)比。結(jié)果:圍絕經(jīng)早期、圍絕經(jīng)晚期、絕經(jīng)后組其E2水平顯著下降(p0.05);FSH水平顯著上升(p0.05);而胰島素抵抗水平(HOMA-IR)隨著絕經(jīng)過(guò)程的進(jìn)展而逐步上升(p0.05);接受HRT后的女性其E2水平較用藥前上升(p0.05),FSH水平較用藥前下降(p0.05),HOMA-IR較用藥前顯著下降(p0.05)。 結(jié)論:隨著女性逐步步入絕經(jīng),其內(nèi)分泌激素水平發(fā)生劇烈變化,表現(xiàn)為E2水平的不斷降低,及FSH水平的顯著升高,同時(shí),隨著絕經(jīng)狀態(tài)的進(jìn)展,胰島素抵抗水平(HOMA-IR)逐漸升高。圍絕經(jīng)期及絕經(jīng)后女性在接受HRT后,E2水平輕度增高,FSH水平降低,同時(shí)其胰島素抵抗水平得到顯著改善。這提示我們,在實(shí)際臨床中,圍絕經(jīng)期及絕經(jīng)后女性內(nèi)分泌水平可能與女性胰島素抵抗存在一定關(guān)聯(lián),HRT可以一定程度上改善胰島素抵抗相關(guān)疾病,圍絕經(jīng)期及絕經(jīng)后婦女若無(wú)禁忌證,則可應(yīng)用HRT預(yù)防胰島素抵抗相關(guān)疾病。
[Abstract]:Aim: to study the relationship between menopausal status and insulin resistance in perimenopausal and postmenopausal women and the improvement of insulin after hormone replacement therapy (HRT). Methods: all the patients came from the menopausal clinic of the affiliated Obstetrics and Gynecology Hospital of Zhejiang University. According to their menstrual conditions, they were divided into early menopausal group, late menopausal group and postmenopausal group. Fasting blood glucose and insulin levels were calculated and the HOMA index was calculated. The indexes were measured again after their HRT6 months. In postmenopausal group, the level of E2 decreased significantly, the level of P0.05FSH increased significantly, while the level of insulin resistance (HOMA-IRR) gradually increased with the development of menopause. The level of E2 in women after HRT was higher than that before treatment, and the level of FSH was lower than that before treatment. HOMA-IR decreased significantly compared with that before treatment. Conclusion: as women step into menopause, the level of endocrine hormone changes dramatically, which is manifested by the decrease of E2 level and the significant increase of FSH level, at the same time, with the development of menopausal state, The level of insulin resistance (HOMA-IRR) increased gradually. The level of E _ 2 in postmenopausal and peri-menopausal women was slightly increased and the level of insulin resistance was significantly improved. This suggests that, in clinical practice, the level of insulin resistance is significantly improved. The endocrine level of women during and after menopause may be associated with insulin resistance in women. HRT can improve insulin resistance related diseases to some extent, if there is no contraindication between peri-menopausal and postmenopausal women, HRT can be used to prevent insulin resistance related diseases.
