Cx43在羊水過少產(chǎn)婦胎膜及胎盤組織中的表達及其意義

【摘要】:目的:通過探討Cx43在特發(fā)性羊水過少、羊水量正常產(chǎn)婦胎膜和胎盤組織中的定性及相對定量表達情況,分析其在羊水過少發(fā)生中的重要地位及其可能的調(diào)節(jié)機制和作用。方法:選取2014年4月至2015年12月在蘭州大學(xué)第一醫(yī)院婦產(chǎn)科經(jīng)剖宮產(chǎn)生產(chǎn)的足月產(chǎn)婦57例為研究對象,羊水過少產(chǎn)婦27例定為實驗組、羊水正常產(chǎn)婦30例為對照組。采取免疫組化SABC方法和RT-PCR技術(shù)分別檢測Cx43蛋白與m RNA在兩組產(chǎn)婦胎膜及胎盤中的表達和定位。結(jié)果:1.采取免疫組化SABC方法發(fā)現(xiàn)Cx43蛋白在兩組產(chǎn)婦胎膜、胎盤中均有表達。2.羊水過少組中Cx43蛋白在胎膜上皮細胞中表達陽性率較對照組明顯下調(diào),差異顯著(其中?2=5.747,P=0.017),Cx43蛋白在胎盤滋養(yǎng)細胞中表達陽性率亦較對照組明顯下調(diào),差異顯著(其中?2=3.986,P=0.046)。3.利用RT-PCR法發(fā)現(xiàn)Cx43m RNA在兩組產(chǎn)婦胎膜、胎盤中均有表達。4.羊水過少組Cx43m RNA在胎膜中的表達是對照組的0.51倍,較之,差異顯著(其中t=15.792,P0.01),在胎盤中的表達為對照組的0.54倍,較之,差異顯著(其中t=14.782,P0.01)。結(jié)論:1.此次研究得出Cx43蛋白及及其m RNA在人類胎膜、胎盤中皆有表達,且主要表達在羊膜上皮細胞、絨毛膜滋養(yǎng)細胞及胎盤血管內(nèi)皮細胞、胎盤合體滋養(yǎng)細胞。2.羊水過少患者的胎膜和胎盤組織中Cx43蛋白及其m RNA表達較正常組均呈下降趨勢。3.Cx43在羊水量過少及羊水量正常產(chǎn)婦胎盤、胎膜中的表達變化可能參與羊水量異常的調(diào)節(jié),提示Cx43可能在羊水膜內(nèi)吸收途徑及母胎液體交換過程發(fā)揮著重要作用。其在胎膜中的表達機制與在胎盤中的表達機制不同,除對水通透功能外可能與細胞凋亡功能有關(guān),具體功能有待進一步研究。4.羊水過少的新生兒Apgar評分與正常足月新生兒的相比有所降低,經(jīng)比較有顯著差異。據(jù)此推測羊水過少可能會影響妊娠結(jié)局,增加圍產(chǎn)兒缺氧及窒息概率。
[Abstract]:Objective: to investigate the qualitative and relative quantitative expression of Cx43 in fetal membrane and placenta of idiopathic oligohydramnios and normal amniotic fluid. To analyze its important role in the occurrence of oligohydramnios and its possible regulatory mechanism and role. Methods: 57 pregnant women who were born by cesarean section in Department of Gynecology and Obstetrics, first Hospital of Lanzhou University, from April 2014 to December 2015, were selected as subjects. 27 cases of oligohydramnios were selected as experimental group, The expression and localization of Cx43 protein and m RNA in fetal membranes and placentas were detected by immunohistochemical SABC method and RT-PCR technique respectively. Results: 1. Cx43 eggs were found by immunohistochemical SABC method. White in the two groups of maternal membranes, The positive rate of Cx43 protein expression in fetal membrane epithelial cells in oligohydramnios group was significantly lower than that in control group. The positive rate of Cx43 protein in placental trophoblastic cells was also significantly lower than that in the control group (P < 0.05). The expression of Cx43m RNA was found in the placenta of both groups by RT-PCR method. The expression of Cx43m RNA in the placenta of oligohydramnios group was 0.51 times as much as that of the control group, and the difference was significant (tr 15.792U P0.01P 0.01), and the expression of Cx43m RNA in placenta was 0.54 times higher than that in the control group, compared with that in the control group, the expression of Cx43m RNA in the placenta was 0.54 times higher than that in the control group. The results showed that Cx43 protein and its m RNA were expressed in human fetal membrane and placenta, and mainly in amniotic epithelial cells, choriotrophoblast cells and placental vascular endothelial cells. Placental syncytiotrophoblast .2.The expression of Cx43 protein and m RNA in membranes and placenta of patients with oligohydramnios showed a decreasing trend compared with the normal group. 3. Cx43 in placenta of oligohydramnios and normal amniotic fluid. The change of fetal membrane expression may be involved in the regulation of amniotic fluid volume abnormality, suggesting that Cx43 may play an important role in the absorption pathway of amniotic fluid and the exchange of maternal and fetal fluid. The expression mechanism of Cx43 in fetal membrane is different from that in placenta. In addition to the function of water permeability, it may be related to the function of apoptosis, and the specific function needs further study .4.The Apgar score of neonates with oligohydramnios is lower than that of normal full-term neonates. It is inferred that oligohydramnios may affect the outcome of pregnancy and increase the probability of anoxia and asphyxia in perinatal infants.
