【摘要】：目的通過檢測Bcl-2、S100A6在良性子宮平滑肌瘤、富于細胞性子宮平滑肌瘤、子宮平滑肌肉瘤中的表達及意義，探討它們在子宮肌瘤發(fā)生發(fā)展中的作用及與臨床病理指標(biāo)的關(guān)系，并可能進一步為臨床鑒別診斷和預(yù)后提供依據(jù)。方法選取良性子宮平滑肌瘤（UL）30例，富于細胞性子宮平滑肌瘤30例，子宮平滑肌肉瘤（LMS）10例，采用免疫組織化學(xué)SABC-9000法分析檢測Bcl-2、S100A6在30例良性子宮平滑肌瘤（UL），30例富于細胞性子宮平滑肌瘤及10例子宮平滑肌肉瘤（LMS）中的表達，并對其表達特點進行分析。 結(jié)果 1.Bcl-2陽性反應(yīng)位于細胞漿，為棕色顆粒：在30例良性平滑肌瘤（UL）中的陽性表達28例（93.3%），在30例富于性平滑肌瘤（CL）中的陽性表達27例（90.0%），在10例子宮平滑肌肉瘤（LMS）中的陽性表達4例（40%）。三組表達率比較有統(tǒng)計學(xué)意義（X2=14.228，P=0.018），UL組和CL組比較無統(tǒng)計學(xué)差異（X2=0.947，P=0.862），UL組和LMS組比較有統(tǒng)計學(xué)差異（X2=12.624，P=0.002），CL組和LMS組比較有統(tǒng)計學(xué)差異（X2=9.732，P=0.012）。 2. S100A6陽性反應(yīng)位于細胞核或細胞漿，為棕色顆粒：在30例良性平滑肌瘤（UL）中的陽性表達9例（30.0%），在30例富于性平滑肌瘤（CL）中的陽性表達17例（56.7%），在10例子宮平滑肌肉瘤（LMS）中的陽性表達9例（90%）。其陽性表達呈遞增趨勢，三組表達率比較有統(tǒng)計學(xué)意義（X2=11.733，P=0.003），UL組和CL組比較有統(tǒng)計學(xué)差異（X2=4.344，P=0.037），UL組和LMS組比較有統(tǒng)計學(xué)差異（X2=8.620，P=0.003），CL組和LMS組比較有統(tǒng)計學(xué)差異（X2=4.596，P=0.032）。 3.Bcl-2、S100A6在UL、CL和LMS中的表達與患者的年齡、腫瘤結(jié)節(jié)的大小無關(guān)（P0.05）。 結(jié)論 1. Bcl-2在良性子宮平滑肌瘤、富于細胞性子宮平滑肌瘤中的表達遠遠高于其在子宮平滑肌肉瘤組織中的表達。提示Bcl-2表達陽性率高的可能在抑制腫瘤進展方面起一定的作用，進一步證實富于細胞性子宮平滑肌瘤是良性腫瘤。 2. S100A6在良性子宮平滑肌瘤、富于細胞性子宮平滑肌瘤、子宮平滑肌肉瘤組織中的表達呈遞增趨勢，S100A6的表達隨子宮平滑肌腫瘤惡性程度的增加而增加，說明富于細胞性子宮平滑肌瘤可能是介于兩者之間的一種腫瘤，有惡變的傾向，推測S100A6在三種肌瘤的鑒別診斷中有一定的輔助價值。 3. Bcl-2和S100A6的異常表達與惡性腫瘤細胞的增殖和分化有密切的關(guān)系，通過檢測Bcl-2和S100A6有可能對子宮肌瘤的發(fā)生發(fā)展以及預(yù)后作出一個有益的判斷，為臨床治療和判斷肌瘤的良惡性、是否保留子宮及判斷預(yù)后提供了一定的理論依據(jù)。
[Abstract]:Objective to investigate the expression and significance of Bcl-2mS100A6 in benign uterine leiomyoma, Cytoleiomyoma and uterine leiomyosarcoma, and to explore their role in the development of uterine leiomyoma and its relationship with clinicopathological indexes. Methods 30 cases of benign uterine leiomyoma, 30 cases of cytosolic leiomyoma, 10 cases of uterine leiomyosarcoma and 10 cases of uterine leiomyosarcoma were selected. Immunohistochemical SABC-9000 method was used to detect the expression of Bcl-2mS100A6 in 30 cases of benign uterine leiomyoma and 10 cases of uterine leiomyosarcoma. The expression characteristics of Bcl-2mS100A6 in 30 cases of benign uterine leiomyoma and 10 cases of uterine leiomyosarcoma were analyzed. Results. 1. Bcl-2 positive reaction was found in cytoplasm, In 30 cases of benign leiomyoma (ULL), 28 cases (93.3%) were positive, 27 cases (90.0%) were positive in 30 cases of leiomyoma, and 4 cases were positive in 10 cases of uterine leiomyosarcoma (LMSs). There was no significant difference between the two groups. There was no statistical difference between the two groups. There was statistical difference between the two groups. The S100A6 positive reaction is located in the nucleus or cytoplasm, For brown granules: in 30 cases of benign leiomyoma (ULL), 9 cases (30.0%), 17 cases (56.7%) and 10 cases of uterine leiomyosarcoma (LMS), respectively. The positive expression showed an increasing trend, and the positive expression was 30.0% in 30 cases of leiomyoma, 56.7% in 30 cases of leiomyoma, and 9 cases in LMS of 10 cases of uterine leiomyosarcoma. There was statistical difference between the three groups in the expression rate of X2X2, 11.733, P0. 003, UL and CL. There was a statistical difference between X2X2, 4. 344, P0. 037, UL and LMS. There was statistical difference between the CL group and the LMS group. There was a statistical difference between the two groups (X24.596P0. 032). 3. The expression of Bcl-2mS100A6 in ULTCL and LMS was not related to the age of patients and the size of tumor nodules. Conclusion. 1. The expression of Bcl-2 in benign uterine leiomyoma and Cytoleiomyoma is much higher than that in uterine leiomyosarcoma, suggesting that the high expression rate of Bcl-2 may play a role in inhibiting the progression of uterine leiomyoma. It is further confirmed that Cytoleiomyoma is a benign tumor. 2. The expression of S100A6 in benign uterine leiomyoma and Cytoleiomyoma was increased with the increase of malignant degree of uterine leiomyosarcoma, and the expression of S100A6 in uterine leiomyosarcoma increased with the increase of malignant degree of uterine leiomyosarcoma. It is suggested that Cytoleiomyoma may be a kind of tumor between them, and it is likely that S100A6 may be helpful in the differential diagnosis of the three kinds of leiomyomas. 3. The abnormal expression of Bcl-2 and S100A6 is closely related to the proliferation and differentiation of malignant tumor cells. The detection of Bcl-2 and S100A6 may make a useful judgment on the occurrence, development and prognosis of uterine leiomyoma. It provides a theoretical basis for clinical treatment and judgment of benign and malignant leiomyoma, whether to preserve uterus and to judge prognosis.
