【摘要】：目的:探討陰式手術(shù)在治療剖宮產(chǎn)疤痕妊娠(CSP)中的臨床價(jià)值。方法:回顧分析2008年1月~2015年12月廣西壯族自治區(qū)人民醫(yī)院婦科收治的90例CSP患者的臨床資料,其中43例行陰式手術(shù),47例行腹腔鏡手術(shù)治療。結(jié)果:患者均順利完成清除病灶和修復(fù)疤痕手術(shù)。陰式組和腹腔鏡組的手術(shù)時(shí)間分別為(58.1±5.0)min、(61.7±7.3)min,兩組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)中出血量分別為(297.0±91.0)ml、(314.0±109.6)ml,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。陰式手術(shù)均由高年資醫(yī)師完成,與腹腔鏡組(高年資39例,低年資8例)比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。陰式組術(shù)前行子宮動(dòng)脈栓塞術(shù)(UAE)14例,與非UAE病例的術(shù)中出血量比較,差異有統(tǒng)計(jì)學(xué)意義[(249.29±49.21)ml vs(299.31±71.01)ml,P=0.023]。兩組均無手術(shù)并發(fā)癥發(fā)生,兩組的術(shù)后平均住院時(shí)間、血β-HCG恢復(fù)正常時(shí)間比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:陰式手術(shù)行病灶清除、修補(bǔ)疤痕治療CSP可行、安全、有效,具有微創(chuàng)手術(shù)的優(yōu)勢(shì),術(shù)者應(yīng)具備良好的陰式手術(shù)技能,值得臨床推廣。
[Abstract]:Objective: to investigate the clinical value of vaginal operation in the treatment of cesarean scar pregnancy. Methods: the clinical data of 90 cases of CSP in gynecological department of Guangxi Zhuang Autonomous region people's Hospital from January 2008 to December 2015 were retrospectively analyzed. 43 cases underwent laparoscopic surgery and 47 cases underwent laparoscopic surgery. Results: all patients successfully completed debridement and scar repair. The operative time in the negative group and the laparoscopic group was 58.1 鹵5.0 min and 61.7 鹵7.3 min, respectively. There was no significant difference between the two groups. The blood loss during operation was 297.0 鹵91.0ml / ml and 314.0 鹵109.6ml / ml respectively, the difference was statistically significant (P 0.05). Compared with laparoscopic group (39 cases with high seniority and 8 cases with low seniority), the difference was statistically significant (P 0.05). 14 cases of uterine artery embolization were performed before operation in the negative group, and the amount of intraoperative bleeding was compared with that in the non UAE cases. The difference was statistically significant [249.29 鹵49.21ml vs(299.31 鹵71.01ml P0.023]. There were no postoperative complications in the two groups. There was no significant difference in the average postoperative hospitalization time and the recovery time of serum 尾 -HCG between the two groups (P 0.05). Conclusion: it is feasible for CSP to be treated by debridement and scar repair. It is safe, effective and has the advantage of minimally invasive surgery.
