【摘要】：正患者女,60歲。主訴:下肢腫脹3年,外陰多發(fā)丘疹1年。現(xiàn)病史:患者3年前無(wú)明顯誘因出現(xiàn)雙下肢腫脹,晨輕暮重,后腫脹加劇,并出現(xiàn)雙下肢增粗及外陰腫脹。1年前發(fā)現(xiàn)外陰數(shù)個(gè)綠豆大丘疹,偶感瘙癢、疼痛,數(shù)量逐漸增多至布滿整個(gè)外陰。為進(jìn)一步診治于2014年4月22日來(lái)我院就診。既往史:5年前確診子宮內(nèi)膜癌,行子宮切除術(shù),術(shù)后行放療及化療。家族史:家族中無(wú)類(lèi)似疾病患者及其他遺傳病史。體格檢查:一般情況好,系統(tǒng)檢查無(wú)異常。皮膚科檢查:雙下肢對(duì)稱(chēng)性增粗,有非凹陷性水腫,會(huì)陰部彌漫性腫脹,其上密集分布直徑0.2~1.0 cm膚色圓形丘疹,觸之較硬,部分呈半透明狀(圖1),表面無(wú)角化、滲出和出血。皮損組織病理檢查:表皮大致正常,真皮淺層可見(jiàn)擴(kuò)張的淋巴管(圖2)。
[Abstract]:The patient is 60 years old. The main complaint: lower limb swelling for 3 years, vulva multiple papules for 1 year. A year ago it was found that the vulva had several large papules of mung bean, occasionally pruritus and pain. The number gradually increased to cover the whole vulva. In order to further diagnosis and treatment, we came to our hospital in April 22nd 2014. Past history: diagnosed five years ago, hysterectomy, Postoperative radiotherapy and chemotherapy. Family history: no similar diseases in the family and other hereditary disease history. Physical examination: general good, no abnormal system. Dermatology examination: bilateral lower limbs symmetry thickening, there is non-concave edema, Diffuse swelling of the perineum with a dense distribution of 0.21.0 cm in diameter on the upper part of the pimples, hard to touch and partially translucent (Fig. 1, without keratinization, exudation and bleeding. Histopathological examination of the lesions: the epidermis is generally normal. Dilated lymphatics can be seen in the superficial dermis (Fig. 2).
