子癇前期患者血清及胎盤中TGF-β1、Endoglin表達(dá)及其相關(guān)性研究

【摘要】:目的:本實(shí)驗(yàn)通過測定子癇前期患者外周血中TGF-β1、s Eng的濃度及胎盤組織中TGF-β1、Endoglin的表達(dá)水平,對比分析子癇前期和正常孕產(chǎn)婦胎盤組織中TGF-β1、Endoglin的表達(dá)及其血清對應(yīng)的TGF-β1、s Eng的含量,研究其相關(guān)性,探討TGF-β1、Endoglin、s Eng在子癇前期病因及發(fā)病機(jī)制中的作用,進(jìn)一步為以后臨床上預(yù)測、診療方面提供新的理論依據(jù)。方法:隨機(jī)選取2014年6月-2014年12月在我院婦產(chǎn)科住院的子癇前期患者60例(其中輕度子癇前期組30例,重度子癇前期組30例),子癇前期的診斷標(biāo)準(zhǔn)參照謝幸主編的第八版婦產(chǎn)科學(xué)。選取血壓正常的晚期孕產(chǎn)婦30例作為對照組。三組患者在年齡、孕周、分娩方式及孕次之間均無顯著性差異。采用雙抗體夾心酶聯(lián)免疫吸附法(ELISA)測定研究對象外周血清中TGF-β1、s Eng濃度。胎盤組織中TGF-β1、Endoglin以免疫組化SP法檢測。將所得的數(shù)據(jù)用SPSS19.0軟件包進(jìn)行分析,計(jì)量資料用均數(shù)±標(biāo)準(zhǔn)差表示,采用方差分析,計(jì)數(shù)資料采用卡方檢驗(yàn)。結(jié)果:1.血清TGF-β1在正常對照組、輕度子癇前期組、重度子癇前期組的濃度分別為673.68±25.16ng/L、930.93±30.34ng/L、1090.88±31.37ng/L,血清s Eng在正常對照組、輕度子癇前期組、重度子癇前期組各組中的濃度分別為24.76±3.39ng/ml、38.15±3.15ng/ml、47.50±3.60ng/ml。隨著病情的加重,TGF-β1/s Eng濃度呈逐漸上升趨勢。與對照組比較,輕度、重度子癇前期組血清TGF-β1/s Eng濃度明顯升高,重度子癇前期組血清中TGF-β1/s Eng高于輕度子癇前期組,且差異均具有顯著的統(tǒng)計(jì)學(xué)意義(p0.01)。2.TGF-β1在正常對照組、輕度子癇前期組、重度子癇前期組TGF-β1的陽性率分別是26.67%、76.67%、93.33%,Endoglin在正常對照組、輕度子癇前期組、重度子癇前期組胎盤中表達(dá)的陽性率分別為30.00%、73.33%、93.33%,從整體局勢可見,病情越重,TGF-β1的表達(dá)陽性率越高,成遞增的趨勢。三組之間兩兩比較顯示:輕度、重度子癇前期組胎盤TGF-β1/Endoglin表達(dá)強(qiáng)于正常對照組,差異具有統(tǒng)計(jì)學(xué)意義(p0.016),而輕度、重度子癇前期組比較,差異無統(tǒng)計(jì)學(xué)意義(p0.016)。3.各組血清中的s Eng與血清中的TGF-β1之間呈正相關(guān),相關(guān)系數(shù)r分別為0.75、0.69、0.72。各組胎盤組織中TGF-β1與Endoglin的表達(dá)有正相關(guān)性,相關(guān)系數(shù)r分別為0.70、0.65、0.72;各組胎盤組織中TGF-β1與血清中的TGF-β1之間有正相關(guān)性,相關(guān)系數(shù)r分別為0.67、0.73、0.71;各組血清中的s Eng與胎盤中的Endoglin也呈正相關(guān),相關(guān)系數(shù)r分別為0.75、0.72、0.71。且差異均具有統(tǒng)計(jì)學(xué)意義(p0.05)。結(jié)論:1.血清TGF-β1、s Eng參與了正常妊娠及子癇前期的發(fā)生過程。且病情越重,血清TGF-β1、s Eng的表達(dá)越高。2.胎盤組織中TGF-β1、Endoglin的過度表達(dá),與子癇前期發(fā)病關(guān)系密切,隨著病情的加重,表達(dá)逐漸增加。3.血清s Eng可能是胎盤組織分泌Endoglin的一種可溶性形式,并通過與TGF-β1之間的相互作用共同參與了血液循環(huán)中血壓的調(diào)控及內(nèi)皮細(xì)胞的損害。
[Abstract]:Objective: to detect the concentration of TGF- 尾 1s Eng in peripheral blood of preeclampsia patients and the expression of TGF- 尾 1 Endoglin in placenta of preeclampsia and normal pregnant women, and to compare the expression of TGF- 尾 1 Endoglin and the content of serum TGF- 尾 1s Eng in preeclampsia and normal pregnant women. To explore the role of TGF- 尾 1 endoglins Eng in the etiology and pathogenesis of preeclampsia. Methods: from June 2014 to December 2014, 60 cases of preeclampsia were randomly selected in our hospital (including 30 cases of mild preeclampsia group, 30 cases of mild preeclampsia group). There were 30 cases of severe preeclampsia group and 30 cases of severe preeclampsia group. The diagnostic criteria of preeclampsia referred to the 8th edition of obstetrics and gynecology edited by Xie Xing. 30 cases of late pregnant women with normal blood pressure were selected as control group. There was no significant difference between delivery mode and pregnancy. Serum TGF- 尾 1s Eng concentration was determined by double antibody sandwich enzyme-linked immunosorbent assay (Elisa), and TGF- 尾 1- Endoglin in placenta tissue was detected by immunohistochemical SP method. Using SPSS19.0 software package to analyze, The measured data were expressed as mean 鹵standard deviation, ANOVA and chi-square test were used. Results: 1. Serum TGF- 尾 1 in normal control group, mild preeclampsia group, and mild preeclampsia group. The concentrations of serum s Eng in the severe preeclampsia group were 673.68 鹵25.16ng / L 930.93 鹵30.34ng / L 1090.88 鹵31.37ng / L, respectively. The concentrations of serum s Eng in normal control group, mild preeclampsia group and severe preeclampsia group were 24.76 鹵3.39ngml / ml 38.15 鹵3.15ngml / ml 47.50 鹵3.60ng / ml respectively. The concentration of TGF- 尾 1 / s Eng in the mild and severe preeclampsia group was significantly higher than that in the mild preeclampsia group. The serum TGF- 尾 1 / s Eng in the severe preeclampsia group was significantly higher than that in the mild preeclampsia group, and the difference was statistically significant (P < 0.01). 2. TGF- 尾 1 in the normal control group and the mild preeclampsia group. The positive rates of TGF- 尾 1 in the placenta of the severe preeclampsia group were 26.677,76.670.33and 93.33 respectively. The positive rates of TGF- 尾 1 expression in the placenta of the mild preeclampsia group, the mild preeclampsia group and the severe preeclampsia group were respectively 30.0073.33 and 93.33.The positive rate of TGF- 尾 1 expression in the placenta of the severe preeclampsia group was higher than that of the severe preeclampsia group. The expression of TGF- 尾 1 / Endoglin in placenta of mild and severe preeclampsia group was higher than that of normal control group, the difference was statistically significant (p 0.016), but that of mild and severe preeclampsia group was higher than that of mild and severe preeclampsia group. There was no significant difference between the two groups. There was a positive correlation between serum s Eng and TGF- 尾 _ 1 in each group, and the correlation coefficient r was 0.75-0.69 ~ 0.72, respectively. There was a positive correlation between the expression of TGF- 尾 _ 1 and Endoglin in placenta of each group, and the expression of TGF- 尾 _ 1 was positively correlated with the expression of TGF- 尾 _ 1 in placenta of each group. There was a positive correlation between TGF- 尾 _ 1 in placenta and TGF- 尾 _ 1 in serum (r = 0.670.73 鹵0.71), and there was a positive correlation between serum s Eng and placenta Endoglin. The correlation coefficient r was 0.75 鹵0.72 鹵0.71, respectively. The differences were statistically significant (P 0.05). Conclusion: 1. Serum TGF- 尾 1s Eng is involved in the development of normal pregnancy and preeclampsia. The more serious the condition, the higher the expression of TGF- 尾 1s Eng. 2. The overexpression of TGF- 尾 1 Endoglin in placenta. The expression of serum s Eng may be a soluble form of Endoglin secretion in placenta. The interaction with TGF- 尾 1 involved in the regulation of blood pressure and the damage of endothelial cells.
